臀牵引术在头位剖宫产术娩头困难中的应用

摘　　要：目的:探讨臀牵引术在剖宫产中胎头娩出困难的应用效果。方法:从2009年1月～2011年1月我院分娩孕妇中选择头位剖宫产80例,胎头娩出困难,其中40例产妇采用产钳助产(产钳组),40例产妇采用臀牵引助产(臀牵引组),观察两组胎儿娩出时间,母婴并发症。结果:臀牵引组胎儿娩出时间短于产钳组(t=19.48,P<0.05),新生儿窒息率低于产钳组(χ2=15.12,P<0.05),术中出血量少于产钳组(t=20.6,P<0.01),新生儿头皮血肿少于产钳组(χ2=15.18,P<0.05)。结论:头位剖宫产胎头娩出困难时采用臀牵引,胎儿娩出时间短,母婴损伤及并发症较少,值得应用。Objective： To explore the effect of hip traction in cesarean section with difficulties of fetal head delivery.Methods： 80 cases of cesarean section with difficulties of fetal head delivery in our hospital from October 2007 to October 2010 were selected,they wer divided into two groups,40 cases with forceps aidstodelivery（forceps group）,40 cases with hip traction aidstodelivery（hip traction group）,the delivery time,complications of mother and child were observed.Results： Delivery time of hip traction group was shorter than that of the forceps group（t=19.48,P0.05）,neonatal asphyxia rate was lower than that of the forceps group（χ2=15.12,P0.05）,blood volume was less than that of the the forceps group（t=20.6,P0.01）,Neonatal scalp hematoma was less than that of the forceps group（χ2=15.18,P0.05）.Conclusion： Hip traction is applied in cesarean section with difficulties of fetal head delivery,delivery time is short,injury and complications of mother and child is fewer,it is worthy of application.
