杭州经济技术开发区流动人口肺结核转出原因及转诊管理效果分析

摘　　要：[目的]分析杭州经济技术开发区流动人口中肺结核患者转出的原因,总结跨区域转诊管理经验,减少因患者转出造成的治疗中断。[方法]对杭州经济技术开发区2007～2008年登记确诊的流动人口肺结核患者相关资料进行分析。[结果]2007～2008年累计登记确诊肺结核患者678例,其中流动人口639例,占94.25%。639例流动人口患者中,转出268例,占41.94%。转出的268例中,转出原因为大专院校学生休学的占37.69%,因经济原因的占26.87%,因不能继续在单位集体生活的占19.03%,被辞退的占13.81%,去向不明的占2.61%。632例流动人口肺结核患者中,成功治疗的550例,成功治疗率为87.03%;户籍人口肺结核患者39例中,成功治疗的34例,成功治疗率为87.18%。户籍人口患者成功治疗率与流动人口患者成功治疗率的差异无统计学意义(P>0.05)。流动人口患者成功治疗率,转出的为76.25%,未转出的为94.61%(P<0.01)。转出流动人口患者成功治疗率,转出原因是大专院校学生休学的为96.04%,因经济原因转出的为48.61%,因不能继续在单位集体生活回原籍者为76.47%,被辞退者为75.68%(P<0.01)。[结论]肺结核病人跨区域转诊管理在学生中实施效果较好,经济困难是影响流动人口治疗转归的重要原因。[Objective]To analyze the causes of transfer during treatment in floating population of tuberculosis patients in HEDA,sum up management experience in cross-regional referral,in order to reduce related treatment interruption.[Methods]Analysis was made on data of registered tuberculosis patients of floating population in HEDA,2007-2008.[Results]Cumulative registration diagnosed tuberculosis were 678 cases duirng 2007-2008,639 of which were floating population,accounting for 94.25%.Duiring treatment,268 patients transferred out of the area occupying 41.94% of the total transient population patient.In the 268 transferred patients,37.69% was college students,26.87% patients transferred for economic reason,19.03% for reason of difficulty in continuing unit collective life,13.81% for being fired and 2.61% missing.550 of 632 registration patients of floating population were treated successfully,successful treatment rate of which was 87.18%.There are 34 patients treated successfully of 39 registration patients of census register population,with the successful treatment rate 87.18%.The difference between them was not statistically significant（P0.05）.The rate of successful treatment of transfer patients and not transfer patients was 76.25% and 94.61%,respectively（P0.01）.The rate of successful treatment of the reason for college students,for economic,for couldn＇t continuing unit collective life and for being fired was 96.04%,48.61%,76.47% and 75.68%（P0.01）.[Conclusion]The effect of tuberculosis patient trans-regional referral management in the college students is the best.Economic difficulties are the important reasons for TB treatment in floating population.
