梅毒患者外周血淋巴细胞CD28-B7分子的表达及意义

摘　　要：目的探讨CD28-B7 T淋巴细胞活化协同刺激分子在梅毒发病、发展及转归中的作用。方法应用流式细胞仪和逆转录-聚合酶链反应(RT-PCR)对60例梅毒患者(早期梅毒患者和血清固定患者各30例)和30例正常人外周血淋巴细胞中B7(CD80/CD86)及CD4+T淋巴细胞中CD28的表达进行检测。结果①梅毒患者外周血淋巴细胞表面CD28的表达水平低于正常对照组,差异有统计学意义(P<0.05);血清固定组和早期梅毒组比较,CD28表达水平的差异无统计学意义(P>0.05)。②梅毒患者外周血淋巴细胞CD80及CD86的表达水平明显低于正常对照组(P<0.05),但CD80和CD86的表达水平在血清固定组和早期梅毒组间无统计学意义(P>0.05)。结论本研究提示梅毒患者外周血淋巴细胞表面CD80、CD86及其受体CD28的表达下调,通过影响CD28-B7协同刺激通路,抑制T细胞尤其是CD4+T淋巴细胞活性功能,可能参与了梅毒的发病、发展及转归过程。Objective To investigate the significance of CD28-B7,co-stimulatory molecules of T lymphocytes in the onset,development and prognosis of patients with syphilis.Methods Flow cytometry and RT-PCR technique were applied to detect the expresstion levels of B7（CD80/CD86）in peripheral blood lymphocytes and CD28 in CD4＋T lymphocytes from 60 syphilis patients（30 seroresistant syphilis patients and 30 early syphilis patients）and 30 healthy volunteers as control group.Results ①The expression of CD28 in patients with syphilis was lower than what in the control group（P0.05）,but the CD28 level in patients with seroresistant syphilis had no difference from that in patients with early syphilis（P0.05）.②The expression of CD80 and CD86 in patients with syphilis was significantly lower than that in the control group（P0.05）,but the CD80 and CD86 level in patients with seroresistant syphilis had no difference from that in patients with early syphilis（P0.05）.Conclusion The expression of CD80,CD86 and CD28 on peripheral blood lymphocytes in patients with syphilis decreased,which impacted the CD28-B7 co-stimulatory pathway and then the immune function of T cells were impaired.This may be involved in the onset,development and prognosis of syphilis.
