GSS椎弓根系统加后路减压融合治疗退变性腰椎滑脱

摘　　要：目的:探讨后路应用通用型脊柱内固定系统(general sp ine system,GSS)复位固定,减压,椎体间植骨融合治疗退变性腰椎滑脱症临床效果。方法:将102例退变性腰椎滑脱症患者经保守治疗无效后采用GSS椎弓根螺钉系统内固定,后路减压融合手术治疗进行回顾性分析,其中Ⅰ度滑脱83例,Ⅱ度滑脱19例。比较手术前后临床症状及X线片。结果:所有患者术后随访均超过6个月。退变性腰椎滑脱患者术后神经功能改善率为85.3%;脊柱活动改善率为89.2%;腰背痛改善率为91.2%;下肢疼痛改善率为92.2%;椎体复位满意率为92.2%。出现并发症共12例,其中术中并发症5例,发生率为4.9%;术后并发症7例,发生率为6.9%。结论:GSS椎弓根螺钉系统能使退变性腰椎滑脱得到较为确实的复位和固定,并为彻底减压提供有效空间,脊柱融合率高,临床效果满意。Objective：To analyze the clinical effect of treatment of degenerative lumber spondylolisthesis with GSS pedicle screw spinal system reduction,fixation,decompression and interbody bone grafting fusion.Methods：From January,2000 to April,2006,102 cases of degenerative lumbar spondylolisthesis were enrolled in this retrospective study.According to Meyerding classification,roentgengram showed that there were： 83cases of grade Ⅰ°,19 cases of grade Ⅱ°.All patients were treated with decompression of half or total laminectomy and nerve canal,interbody fusion was done with GSS pedicle screw spinal system when the expectant treatment is ineffective.Results：All the patients were followed up over 6 months.In the patients with degenerative spondylolisthesis,the amelioration of nerve function was 85.3 %;the improvement of spinal motion was 89.2%;the allevioration of low back pain was 91.2 %;the allevioration of leg pain was 92.2 %;the mean reduction rate of degenerative spondylolisthesis was 92.2 %.Complications occurred in twelve cases,in which five cases occurred during operation with the incidence of 4.9 %,while the other seven occurred after operation with the incidence of 6.9%.Conclusion：GSS pedicle screw spinal system can distract the slipped vertebrae effectively to provide the sufficient space for the complete decompression with the satisfactory reduction and the strong fixation strength and provide relately high fusion rate.
