骨性指标测量在截石位手术中的应用

摘　　要：目的探讨通过骨性指标测量指导手术中放置截石位的可行性。方法将2009年9月至2010年3月进行截石位手术的患者83例随机分为对照组和试验组:对照组采用常规方法摆放截石位;试验组在测量患者的股骨长度,角尺测量两大腿间、小腿与大腿间、搁脚板与搁脚架之间角度的基础上,根据测量结果指导截石位肢体位置的摆放。比较两组患者体位摆放后即时患者及手术医生的满意度,以及术后3 d患者的舒适度。结果术后即时,试验组患者和医生的满意度均高于对照组,但差异尚无统计学意义(P>0.05);试验组患者术后3 d出现脚麻、活动受限和皮肤受压的比例均低于对照组,差异均有统计学意义(P<0.05)。结论通过骨性指标的测量,为截石位的摆放提供了科学依据,有利于减少术后并发症,提高患者和手术医生的满意度。Objective To study the feasibility of using index measurement of bone to direct the setting of lithotomy position for surgery patients.Methods From September 2009 to March 2010,83 surgery patients who underwent lithotomy position surgery were divided into a control group to undergo the conventional method of setting lithotomy position and an experimental group to undergo the setting of lithotomy position according femur length,the degree of the angle between two thigh,the angle between thigh and shank and the angle between toe board and foot supporter shelf.Comparisons were conducted on the satisfaction rates of surgeons and patients to the body position and patients＇ comfort levels in 3 d after surgery in the two groups.Results The post-operative satisfaction rates of surgeons and patients were higher in the experimental group than those in the control group without statistical significance（P0.05）.In the 3rd postoperative day,the occurrence rates of feet numbness,limited mobility and skin pressure were significantly higher in the experimental group than those in the control group（P0.05）.Conclusion Index measurement of bone can be used to direct the setting of lithotomy position for providing scientific evidence for lithotomy position,reducing postoperative complications and improving the satisfaction of surgeons and patients to body position.
