重组甘精胰岛素与低精蛋白胰岛素联合格列美脲治疗2型糖尿病的疗效观察

摘　　要：目的观察重组甘精胰岛素(长秀霖)与低精蛋白胰岛素(诺和灵30R)联合格列美脲治疗2型糖尿病的有效性和安全性。方法将60例糖尿病患者随机分为长秀霖组和诺和灵30R组,每组30例。长秀霖组每天晚上18∶00点注射长秀霖,诺和灵30R组每天早、晚餐前2次皮下注射诺和灵30R,2组每天早餐前30mim均口服格列美脲3mg。观察2组患者空腹血糖(FPG)、平均餐后2h血糖(2hPG)、糖化血红蛋白(HbA1C)、C肽水平及低血糖事件的发生率。结果 2组治疗后FPG、2hPG、HbA1C水平均有明显下降(P<0.05),但2组间比较差异无统计学意义(P>0.05);且长秀霖组治疗后C肽升高(P<0.05),且于高于诺和灵30R组C肽水平(P<0.05)。长秀霖组低血糖事件发生率为6.7%低于诺和灵30R组的23.3%(P<0.05)。结论该2种治疗方法的疗效大致相同,但长秀霖联合格列美脲治疗2型糖尿病低血糖事件发生率低,并有可能改善β细胞功能,故更具有选择优势。Objective To observe the effectiveness and safety of recombinant insulin glargine（Changxiulin） ,biosynthetic human insulin（Novolin 30R） combined with glimepiride in type 2 diabetes mellitusn.Methods 60 cases patients with diabetic were divided into Changxiulin group and Novolin 30R group,each of 30 cases.Changxiulin group injected Changxiulin at 18∶ 00 every night,Novolin 30R group subcutaneous injected Novolin 30R before breakfast and supper,two groups were combined glimepiride 3mg oral before breakfast 30mins.Observed the level of fasting plasma glucose（FPG） ,average 2h postprandial glucose（2hPG） ,glycated hemoglobin（HbA1C） ,C peptide and incidence rate of low blood sugar event of 2 groups.Results After treatment,the level of FPG,2hPG,HbA1C of 2 groups were significantly decreased（P0.05） .After treatment,the level of C peptide of Changxiulin group increased（P0.05） ,and was higher than that of Novolin 30R group（P0.05） .The incidence rate of hypoglycemic events of Changxiulin group（6.7%） was lower than that of Novolin 30R group（23.3%）（P0.05） .Conclusion The treatment methods of 2 kinds are similar,but Changxiulin combined with glimepiride in type 2 diabetes mellitusn has a low incidence of hypoglycemia,and may improve β-cell function.
