北京地区健康儿童PT和APTT正常参考范围的探讨

摘　　要：目的 建立北京地区儿童血浆凝血酶原时间（plasma prothrombin time,PT）和活化部分凝血活酶时间（activated partial thromboplastin time,APTT）的正常参考值范围,为临床诊治提供依据.方法 采用法国STAGO全自动血凝分析仪检测北京地区1 111例正常儿童的PT和APTT.将所有儿童根据性别不同分成男（582例）、女（529例）两组;根据发育情况分为婴儿组（257例）、幼儿组（265例）、学龄前组（271例）、学龄组（192例）、青春发育期组（126例）.采用美国临床实验室标准化协会推荐的C28-A2文件中提供的关于临床实验室如何确定和建立生物参考区间的方法对所有数据进行分析.结果 经过正态性检验,儿童PT和APTT检测值均呈正态分布.经Z检验后发现不同性别儿童的APTT分布差异无统计学意义（t=1.98,P〉0.05）,但PT分布差异有统计学意义（t=0.837,P〈0.05）.不同年龄段儿童的PT和APTT差异存在统计学意义 （FAPTT=5.665,P〈0.01;FPT=5.755,P〈0.01）.根据年龄和性别不同,计算APTT和PT的参考范围为：青春发育前组：PT（男）11.9～14.6 s,PT（女）12.0～14.7s;APTT 29.6～50.7 s.青春发育组：PT（男）11.3～14.6 s,PT（女）11.6～14.6 s;APTT 26.5～46.6 s.结论 初步建立北京地区儿童PT和APTT的正常参考范围,对于临床诊治和筛查凝血障碍性疾病、血栓性疾病以及临床使用抗凝剂监测等方面具有十分重要的意义.Objective To determine reference intervals of plasma prothrombin time （PT）and activated partial thromboplastin time（APTT） of 1 111 healthy children in Beijing region in order to provide evidences for clinic. Methods The plas- ma samples of 1 111 healthy children were detected by STA Compact system. The children were divided into 2 groups （male 582 cases ;female 529 cases） according to gender ;The children were divided into 5 groups（infancy group 257 cases ; toddler＇s age group 265 cases ;preschool age group 271 cases ;school age group 192 cases ;adolescence group 126 cases） according to ages. The reference intervals of PT and APTT were determined by the method of C28-A2 recommend by Clinical and Laboratory Standards Institute. Results According to the results of test of normality the levels of PT and APTT of children belonged to normal distribution. There were not obvious significance of APTT in different gender groups （t= 1.98,P〉0. 05）. There were obvious significance of PT in different gender groups （t=0. 837 ,P〈0. 05）. There were obvious significance of PT and APTT in different age groups （FAPTT = 5. 665 ,P〈0. 01 ;FPT= 5. 755,P〈0. 01）. Determined reference intervals of APTT according to different age groups and determine reference intervals of PT accordingto different age groups and gender groups. Before adolescence group：PT（male）11. 9-14.6s,PT（female）12.0-14. 7s;APTT 29.6- 50. 7s. Adolescence group：PT（male）11. 3- 14.6s,PT（female）11. 6-14.6s;APTT 26.5-46.6s. Conclusions It is very necessary for clinical diagnosis and treatment of hemorrhagic disease by determining reference intervals of PT and APTT.
