摘 要:目的 探讨血浆(1,3)-β-D-葡聚糖(BG)检测(G试验)对老年人深部真菌感染的诊断价值.方法 采用动态比浊法检测67例老年高危患者的血浆BG浓度,并与真菌培养结果进行比较.结果 67例高危患者,G试验31(46.27%)例阳性,真菌培养法21(31.34%)例阳性.深部真菌感染组血浆BG中位数浓度为45.97 pg/ml;非真菌感染组浓度为5.00 pg/ml,两组差异有统计学意义(Z=-6.138,P<0.01).若以真菌培养为标准,G试验的敏感度、特异度、阳性预测值和阴性预测值分别为90.48%,73.91%,61.29%和94.44%.结论 G试验对于老年人深部真菌感染具有早期、快速的诊断价值.Objective To analyse the diagnostic value of plasma (1,3)-β-D-glucan (BG) assay (G-test) for oldsters who suffer deep fungal infection. Methods The concentrations of serum BG were detected with the kinetic turbidimetric method in 67 elderly high-risk patients,and compared with the results of fungal culture. Results There were 31(46. 27%) positive samples identified by G test and 21(31.34%) positive samples identified by fungal culture in 67 patients who were suspected deep mycosis. The median concentration of serum BG in the deep fungal infection group was 45.97 pg/ml,while it was 5.00 pg/ml in the non-infected group. There were statistically differences between the two groups (Z =- 6. 138,P〈 0. 01). When fungal culture was applied as the standardization of diagnosing deep fungal infection,the sensitivity,specificity,positive and negative predictive values of G-test were 90. 48%, 73.91% ,61.29% and 94. 44% ,respectively. Conclusion G-test is a valuable method which could rapidly diagnose the deep fungal infection in the elderly in the early phase.
