超声引导下乳腺可疑病灶穿刺活检的临床价值

摘　　要：目的:在超声引导下乳腺病灶穿刺活组织病理学检查临床应用价值。方法:回顾性分析比较我院60例超声引导下穿刺乳腺病灶穿刺活检标本和手术后标本的病理组织学诊断。结果:送检的超声引导下乳腺病灶穿刺活检标本的病理组织学诊断分别为:浸润性癌36例,髓样癌2例,可疑恶性3例,非典型增生4例,叶状肿瘤1例,良性病变14例。超声引导下乳腺病灶穿刺活检与手术后活检病埋诊断符合率为97%,Kappa=0.921(P<0.01)。结论:超声引导下乳腺病灶穿刺活检安全可靠,它在乳腺癌早期诊断及合理治疗中有重要的临床应用价值。Objective：To evaluate the role of ultrasound-guided core-needle biopsy（CNB）of breast lesions.Methods：Data were collected retrospectively from 60 ultrasound-guided CNB that were followed by operation.The histological findings obtained at ultrasound-guided CNB and operation were identified for each category and consistency test was done.Results：The sixty lesions,ultrasound-guided CNB identified invasive breast carcinoma in thirty-six,cephaloma in two,suspicious malignancy in three,atypical hyperplasia in four,phyllodes turnout in one,benign in forteen.Histopathologic examination results at ultrasound-guided CNB were confirmed at operation for 97%,generating a Kappa value of 0.921（P0.01）.Conclusion： Ultrasound-guided core needle biopsy is a safe and reliable diagnostic method,which play an important role in early diagnosis and rational therapy of breast carcinoma.
