连续性血液净化治疗在重度烧伤脓毒症患者中的临床应用

摘　　要：目的探讨应用连续性血液净化疗法(CBP)对严重烧伤脓毒症患者的疗效。方法 28例重度烧伤脓毒症患者随机分为血液净化组和常规治疗组,每组各14例,两组均经常规治疗,血液净化组在常规治疗的基础上进行连续性血液净化治疗。观察两组患者治疗前后生命体征体温、心率、血压、呼吸频率的变化,并观察两组患者治疗前后实验室检查,包括血常规、电解质、血糖(GLU)、血尿素氮(BUN)、血肌酐(Cr)检测及白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)浓度检测。结果血液净化组患者治疗后生命体征明显改善,血液净化组治疗后血常规、电解质、BUN、Cr、IL-8、TNF-α的数据较常规治疗组治疗后有明显下降,差异有统计学意义(P<0.05)。结论连续性血液净化疗法能够降低炎性介质,改善脏器功能,从而改善烧伤脓毒症患者的反应过程及预后,是治疗严重烧伤脓毒症的有效措施之一。Objective To evaluate the effect of continuous blood purification therapy on severely burned patients with sepsis. Methods M1 the 28 cases of severe burn sepsis were randomly divided into blood purification group and routine treatment group. Each of the 14 patients in the control group accepted conventional treatment, while the experimental group accepted continuous blood purification therapy. Vital signs including body temperature, heart rate, blood pressure, respiratory rate were observed before and after treatment. Laboratory tests including blood routine, electrolytes, blood glucose （ GLU）, blood urea nitrogen （BUN）, blood ereatinine （Cr） and interleukin 8 （IL-8）, tumor necrosis factor α （TNF-α） were also observed. Results Vital signs of blood purification therapy patients significantly improved, blood purification, electrolytes, BUN, Cr, IL-8, TNF-α decreased significantly after treatment compared the conventional treatment group, the difference being statistically significant （P 〈 0.05 ）. Conclusions Continuous blood purification can not only reduce inflammatory mediators, but also improve organ function and the reaction process of burn patients with sepsis and prognosis. The treatment of severe burn with sepsis is one of effective measures.
