摘　　要：目的　报告在不使用或有限使用 C臂 X线监视下应用 Richards钉内固定治疗股骨粗隆间骨折的经验与教训 ,并对本方法的治疗结果进行评估。方法　 3 0例股骨粗隆间骨折均采用“指触定位”法行 Richards钉内固定手术。对不稳定型骨折应用“螺钉去压”等方法加强固定牢度。结果　稳定型骨折负重时间为术后 1周 ,不稳定型骨折为术后 3周。随访 5～ 2 8月 ,平均愈合时间 1 0周。关节功能优良 2 7例 ( 90 %)。结论　该方法具有操作方便、复位准确、固定可靠、X线伤害小、允许早期负重、功能恢复快、并发症少等优点。手术操作中应强调骨折复位的重要性和避免使用电动绞刀扩孔。Objective To report the experience in treatment of intertrochanteric fracture with Richards nailing under X ray or not and evaluate their therapeutic results.Methods 30 cases of intertrochanteric fracture were fixed internally with Richards nailing according fo “finger touch feeling orientation”.For instable fractures,the “decompression screw” or other methods were applied to make reliable fixation.Results The weight loading for stable fracture could begin one week after operation and that for instable fracture three weeks after operation.By following-up for 5-28 months,the fracture healing period was 10 weeks in average.The function of hip joint was excellent in 27 cases (90%).Conclusion There are distinguished advantages in This method,such as simple application,reliable fixation,early weight bearing quick recovery of function,and only few complications and less X-ray damage.The importance of fracture reduction and the avoidance from electrical drilling in operation are emphasized.
