摘  要:目的分析比较血清胱抑素C、尿素氮和肌酐在糖尿病肾损害诊断中的临床价值。方法收集健康对照组51例,单纯糖尿病患者48例,糖尿病早期肾损害患者60例,糖尿病中度肾损害患者40例,糖尿病重度肾损害患者29例。所有入选者均用免疫比浊法测定血清胱抑素C,用酶法测定尿素氮和肌酐。将各实验组的检测均值与健康对照组进行比较分析其浓度变化是否具统计学意义,并将血清胱抑素C的阳性率与同组的尿素氮和肌酐的阳性率进行比较分析阳性检出率是否具统计学意义。结果各组的血清胱抑素C的浓度均值与健康对照组比较均有统计学意义(P<0.05),尿素氮和肌酐的浓度均值中仅有中度肾损害组和重度肾损害组与健康对照组比较有统计性意义(P<0.05)。在单纯糖尿病组,早期肾损害组和中度肾损害组血清胱抑素C的阳性率与尿素氮和肌酐阳性率比较有统计学意义(P<0.05)。结论在糖尿病肾病诊断指标中,血清胱抑素C是更敏感,更可靠的指标。Objective To explore the clinical diagnosis value of Cys C,BUN and Cr in diabetic kidney harm.Methods Collect patient'serum sample of 51 healthy control,48 pure diabetes,60 diabet with mild kidney harm,40 diabetes with moderate kidney harm,and 29 diabetes with serious kidney harm,then determine the value of Cys C by immunity turbidimetric method,and determine the value of BUN and Cr by enzyme method.Analyse the value of each diabetic groups by contrast with the heathy group,and also analyse the diagnostic value about the positive rate of the Cys C,BUN and Cr in each groups.Results Compared with the healthy control,the changes of the Cys C value of the other diabetic groups have statistics significance(P0.05),but the changes of the BUN and Cr value only in the groups of diabetes with moderate and serious kidney harm have statistics significance(P0.05).In the groups of the pure diabetes,diabet with mild kidney harm and diabetes with moderate kidney harm the positive rate of the Cys C compared with the positive rate of BUN and Scr have statistics significance(P0.05).Conclusions The Cys C is more sensitive and reliable indicator in diagnosis of diabetic nephropathy.
