摘 要:目的研究血清游离脂肪酸水平在国际糖尿病联盟(IDF)代谢综合征诊断标准分组患者中的变化和利用腰围、血压等标准评价代谢综合征患者胰岛素抵抗程度的可能性。方法按IDF代谢综合征标准,根据腰围、血压、血糖及血脂水平,将研究对象分为正常对照组、轻度代谢异常组和重度代谢异常组,测定空腹血清血糖、胰岛素、游离脂肪酸(FFA)和三酰甘油等水平,根据稳态模式评估法计算胰岛素抵抗指数HOMA-IR,分析各组HOMA-IR的变化及FFA和胰岛素抵抗的关系。结果和正常组比较,代谢异常组HOMA-IR和FFA水平明显升高,其中轻度组FFA为正常组的2.10倍,而重度组FFA达到正常组的2.65倍。相关性研究显示:腰围、收缩压、舒张压、三酰甘油、HOMA-IR和血清FFA呈明显正相关。结论直接测定腰围、血压、血糖和三酰甘油是简单有效的评价体内胰岛素抵抗程度的方法;游离脂肪酸可以间接反映代谢综合征患者胰岛素抵抗的程度。【Objective】 To investigate the levels of serum free fatty acids among the grouped metabolic syndrome subjects diagnosed by the International Diabetes Foundation(IDF) criteria,and to survey the possibility of using waist circumference and blood pressure to evaluate the insulin resistance degree of metabolic syndrome subjects.【Methods】 The subjects are divided into 3 groups: normal control group,low-grade metabolism abnormal group,high-grade metabolism abnormal group according to the IDF metabolic syndrome diagnosis criteria.The levels of fasting serum glucose,fasting serum insulin,FFA and triglyceride were measured.HOMA-IR was calculated by the homeostasis model assessment.【Results】 Compared with the normal group,the levels of HOMA-IR and FFA in metabolic syndrome subjects increased significantly.The serum FFA of low-grade and high-grade metabolic abnormal subjects increased by 2.10 and 2.65 times respectively compared with the normal subjects.There were marked positive correlations between waist circumference,systolic blood pressure,diastolic blood pressure,triglyceride,HOMA-IR and serum FFAs.【Conclusion】 Measuring waist circumference,blood pressure,serum glucose and serum triglyceride is simple and effective method to evaluate the insulin resistance.Free fatty acids could be the possible marker which reflects the insulin resistance degree of metabolic syndrome subjects.
