急进高原人群不同海拔梯度暴露对尿液成分的影响

摘　　要：目的了解平原人群移居不同海拔高度尿蛋白变化规律,进一步探讨高原低氧环境对肾脏的影响。方法随机选取32名进入高原的士兵作为观测对象,在进入高原前和进入高原途中每日晨起留取中段尿10mL,连续检测4 d,到达后用BAYER尿10项分析仪和试纸进行定性和定量检测。结果移居高原途中尿液成分有明显变化,反应最突出的是尿蛋白,进入高原当日随海拔梯度增高,尿蛋白阳性率增多,当海拔高度变化不明显时,尿蛋白的阳性率有所减少;海拔再度升高,尿蛋白再度增多;再次平缓下降时,尿蛋白又有所减少。尿酮体随进驻时间的延长而逐步增多;尿潜血、亚硝酸盐、白细胞、胆红素随进入海拔的高度和时间略有改变。结论高原低氧环境下暴露尿蛋白出现迅速且与进入高原的梯度成正相关,尿酮体与进驻高原的时间成正比。【Objectives】 To explore variations on urine proteins of healthy young men who migrated to different altitude,so as to investigate the effects of hypoxic environment on kidney.【Methods】A total of 32 healthy young men,who migrated to high altitude were investigated.10 mL morning midstream urine was col-lected before entering and on the way to plateau.Samples were continually detected for 4 days by BAYER urine ten items analyzer and test paper.【Results】There was a marked change in urine composition of mi-grants on the way to plateau and the dramatic change was urine protein.On the day arrived plateau,positive rate of urine protein significantly increased with the elevating altitude gradient and step down with no obvious change in altitude gradient,increased when elevating again,and vice versa.Urine acetone bodies increased gradually along with the inhabited time while occult urine test,urinary nitrite,WBC and bilirubin changed slightly with inhabited altitude and time.【Conclusion】There was a positive correlation between positive rate of urine proteins and altitude gradient,urine acetone bodies were proportionally associated to time in road en-tering plateau.
