腹腔镜治疗输卵管积水性不孕64例分析

摘　　要：目的分析腹腔镜下输卵管造口术、粘连松解术治疗输卵管远端积水不孕症的妊娠结局,评价其疗效。方法 2006年6月～2009年7月经子宫输卵管碘油造影术证实输卵管积水64例,均行腹腔镜检查,诊断明确后行输卵管造口术、粘连松解术。分为输卵管积水直径<3cm和输卵管积水直径≥3cm两组,通过术后随访,观察两组妊娠情况。结果 64例患者,妊娠17例,妊娠率26.56%;输卵管积水直径<3cm有36例,妊娠15例,妊娠率41.67%,输卵管积水直径≥3cm有28例,妊娠2例,妊娠率7.11%,两组间差异有显著性(P<0.01)。结论腹腔镜输卵管造口术治疗输卵管积水具有优势,输卵管积水直径≥3cm的治疗效果差。[Objective] To investigate postoperative pregnancy rate of infertility patients who had undergone laparoscopic salpingostomy and adhesion with distal hydrosalpinx.[Methods] Sixty-four cases of hydrosalpinx infertility were diagnosed by hystero-salpin-gography from June 2006 to July 2009,treated by laparoscopic salpingostomy.The patients were divided into two groups according to diameter of hydrosalpinx：diameter of hydrosalpinx3 cm and diameter of hydrosalpinx≥3 cm.All the patients were followed up and the results after laparoscopic therapy were observed.[Results] In sixty-four patients,pregnancy existed in 17 cases.The total pregnancy rate was 26.56%.The pregnancy rate of the group of diameter of hydrosalpinx 3 cm was 41.67%,and the pregnancy rate of the group of diameter of hydrosalpinx ≥ 3 cm was 7.11%.There was significant difference in pregnancy rate between 3 cm group and ≥3 cm group （P 0.01）.[Conclusions] The treatment of distal hydrosalpinx by laparoscopic salpingostomy is effective and safe,but the effectiveness for the hydrosalpinx whose diameter of over 3 cm is the lowest.
