围绝经期综合征中医肝郁分级与舌苔脱落细胞凋亡的相关性

摘　　要：目的探讨围绝经期综合征中医肝郁分级与舌苔脱落细胞调控基因的相关性。方法选取50例健康妇女作为对照组,150例围绝经期综合征患者作为观察组,采用证素辨证进行中医肝郁病理分级,巴氏染色观察舌苔脱落细胞的成熟指数(MI)、成熟价值(MV),流式细胞仪检测细胞凋亡指数,免疫组化检测舌苔脱落细胞调控基因Fas、Bax、Bcl-2的阳性率。结果观察组肝郁积分明显高于对照组(P<0.01);观察组肝郁分级分布与对照组有明显差异(P<0.01)。肝郁分级与舌苔脱落细胞MI的中层细胞数量、凋亡指数、Fas阳性率、Bax阳性率呈正相关(P<0.01),与舌苔脱落细胞MI的表层细胞数量、舌苔脱落细胞MV、Bcl-2阳性率呈负相关(P<0.01)。结论肝郁是围绝经期综合征的核心病机之一,肝郁分级与舌苔脱落细胞的成熟度、凋亡指数及凋亡调控基因相关。Objective To study the correlation between TCM liver depression grade and apoptotic regulatory genes of tongue exfoliated cells in perimenopausal syndrome. Methods Fifty healthy women were enrolled into the control group and 150 cases of perimenopausal syndrome attributed to the observed group. The patients were diagnosed with the syndrome-element differentiation for liver depression grading. Tongue exfoliated cell maturation index （MI） and maturation value （MV） were examined by Papanicolaou staining,apoptosis index was detected through flow cytometer,and positive rate of apoptotic regulatory genes Fas,Bax,and Bcl-2 of tongue exfoliated cells were investigated via immunohistochemistry. Results The liver depression score of the observed group was significantly higher than that of the control group （P0.01）. The distribution of liver depression of the observed group was significantly different from that of the control group （P0.01）. Liver depression was positively correlated with the amount of middle layer cells of MI of tongue exfoliated cells,tongue exfoliated apoptosis index,and the positive rate of Fas and Bax of tongue exfoliated cells （P0.01）; while it was negatively correlated with the amount of surface layer cells of MI of tongue exfoliated cells,MV of tongue exfoliated cells,and the positive rate of Bcl-2 of tongue exfoliated cells （P0.01）. Conclusion Liver depression is one of the core pathogenesis in perimenopausal syndrome. Degree of liver depression is correlated with maturity,apoptosis index,and apoptotic regulatory genes of tongue exfoliated cells.
