宁夏2008年死因谱与疾病负担研究

摘　　要：目的调查2008年宁夏死因谱与疾病负担情况。方法对2008年宁夏死因监测资料和全区各级医疗机构《医院出院病人调查表》进行整理和统计分析。结果 2008年全区共报告死亡病例15210例。其中,男性占62%,女性占38%。男、女主要死因均为慢性病,占所有死因77.9%;女性慢性病死亡比例高于男性,男性伤害死亡的比例高于女性;65岁以上年龄组占56.3%;30岁以下年龄组(除0～岁组)因伤害死亡所占比例最大;而在40岁组以上年龄组,则因慢性病死亡占最大比例。前5位的死因顺位依次是循环系统疾病、恶性肿瘤、伤害、呼吸系统疾病和消化系统疾病。按照人均潜在减寿年数(PYLL)排位,则以伤害居首位(45.8年)。在各种死亡原因中因伤害引起的死亡人群平均死亡年龄最小。人均住院费用以恶性肿瘤为高,为13112.76元,其次是伤害和循环系统疾病,在万元以下。结论循环系统疾病仍居死因第一位,恶性肿瘤已经成为第二位死因威胁着人群建康。伤害和恶性肿瘤造成人群健康负担严重并且住院费用高,应该高度重视伤害和恶性肿瘤的预防与控制。Objective To analyze death spectrum and burden of disease of Ningxia in 2008 and to understand the burden of disease caused by death changes in recent years of Ningxia through comparison with literature material.Methods The data of death cause monitoring and the report form of all levels of Medical establishment of Ningxia in 2008 were collected and analyzed.Results Totally 15210 dead cases were reported in 2008.The male accounted for 62% and the female accounted for 38%.The main dead cause was chronic illness,which accounted for 77.9% in all causes.The female chronic illness death proportion was higher than the male one,but the male injury death＇s proportion were higher than the female one.The age group above 65 years occupied 56.3% of all death cases.The main death cause was injury under 30 years old age group,but above 40 years old group,the chronic illness cause took main part.The first 5 death causes in turn were the circulatory disease,the malignant tumor,the injury,the respiratory disorder and the digestive disease.The injury was the first according to an average per person PYLL row of position（45.8 year）.The average death age was the smallest caused by injury in each kind of cause.Average expenses per person in hospital of malignant tumor were the highest and they was 13112.76 Yuan,followed by injury and circulatory disease,which bellow 10,000 Yuan.Conclusion The Ningxia residents health standard and the health service level had the big enhancement in recent years.Circulatory system illness still occupied the first cause of death,the malignant tumor already became the second cause of death to threaten residents health.the health burden caused by the injury and the malignant tumor was serious.
