凝聚法检测D-二聚体在诊断急性肺栓塞中的临床应用

摘　　要：目的评价凝聚法检测D-二聚体试剂在排除急性肺栓塞的效能。方法采用自身对照的试验设计方法,将91例需排除急性肺栓塞病例使用英国BB International公司D-二聚体检测试剂盒(试验试剂)和上海捷门公司的D-二聚体试剂盒(对照试剂)分别对血液样本进行比较测定D-二聚体,对上述两种试剂检测结果不符的标本用第三方试剂盒(胶体金法)进行验证,试验结果判断以两者一致者为准,观察两组阳性符合率、阴性符合率及敏感度和特异度指标。结果试验试剂检出阳性28例,阴性63例;阳性符合率100.0%,阴性符合率84.0%;对照试剂盒阳性16例,阴性75例;以第三方试剂盒进行验证,试验试剂的敏感性和特异性,敏感性为100.0%,特异性为96.9%。结论凝聚法试验试剂适于进行排除急性肺栓塞快速辅助诊断。Objective To evaluate the efficacy of D-dimer coagulation test reagent in the exclusion of acute pulmonary embolism（APE）.Methods A Self-controlled trial design was adapted.Totally 91 patients who needed an exclusion of APE were tested with British BB International D-dimer test kit（trial reagent） and Shanghai Jiemen′s D-dimer kit（control reagent） respectively.If the results on the two reagent samples were inconsistent,the third kit（colloidal gold method） would be used to verify the correctness.And the positive and negative coincidence rates,as well as the sensitivity and specificity of the two groups were observed.Results BB International reagent detected 28 positive cases and 63 negative cases;with a positive consistent rate of 100.0% and a negative coincidence rate of 84.0%;Shanghai Jiemen reagent detected 16 positive cases and 75 negative cases.According to the verification by the third kit,the sensitivity and specificity of BB International reagent were 100.0% and 96.9% respectively.Conclusion D-dimer coagulation test reagent is suitable for rapid auxiliary diagnosis for exclusion of acute pulmonary embolism.
