干扰素抗体与国产重组干扰素治疗慢性乙型肝炎疗效关系

摘　　要：目的　观察应用国产基因重组干扰素α２ｂ 治疗慢性乙型肝炎后干扰素抗体的产生对疗效的影响。方法　病例组予以国产重组干扰素α２ｂ 治疗，随访治疗前后不同时间干扰素抗体产生及患者转归情况；采用酶联免疫法检测结合抗体（ＢＡ） ，抗病毒中和法检测中和抗体（ＮＡ） 。结果　对照组（正常供血者）均未检出ＢＡ 和ＮＡ；而１２０ 例患者接受干扰素治疗后，９ 例血中检出ＢＡ（８ ％ ），５ 例血中检出ＮＡ（４％ ）。治疗结束时，９ 例ＢＡ 阳性患者中３ 例ＨＢＶ ＤＮＡ 阴转（３３ ％ ），１１１ 例ＢＡ 阴性患者中５６ 例ＨＢＶ ＤＮＡ 阴转（５０％ ）（Ｐ＞ ０－０５） ；５ 例ＮＡ 阳性患者中ＨＢＶ ＤＮＡ 均未阴转（０），１１５ 例ＮＡ 阴性患者中５８ 例ＨＢＶ ＤＮＡ 阴转（５０％ ）（ Ｐ＞０－０５） 。结论　结果未显示干扰素抗体是影响重组干扰素α２ｂ 疗效的主要因素。Objective To study influence of interferon (IFN) antibody production on the therapeutic effect of patients with chronic hepatitis B treated with domestic recombinant IFN α 2b. Methods The polycentral study was done. According to the standards and methods set up in advance, the population of patisnts and control groups were chosen. Production of IFN antibodies and prevalence of changes of patients before and after the treatment of IFN were followed up respectively. Enzyme immunoassay (EIA) and antiviral neutralization bioassay (ANB) were used to detect binding and neutralizing antibodies (BA and NA) respectively. Results BA and NA were not found in 100 healthy donors. Among 120 patients treated with recombinant IFN α 2b, BA and NA were present in 9(8%) and 5(4%) ones respectively. At the end of IFN therapy, HBV DNA was undetectable in 3 of 9 patients (33%) with BA and in 56 of 111 ones (50%) without BA( P >0 05).HBV DNA was still detectable in 5 patients (0) with NA and undetectable in 58 of 115 ones (50%)without NA ( P >0 05). Conclusion IFN antibodies (BA and NA) are not main factors affecting the response to recombinant IFN α 2b therapy.
