重症脑出血患者术后早期肠内营养的临床研究

摘　　要：目的 探讨早期实施肠内营养支持对重症脑出血患者术后营养状态及并发症的影响.方法 对42例重症脑出血患者（GCS评分≤8分）随机分为采用留置胃管早期实施肠内营养支持（治疗组）组和采用传统的延迟性胃肠内营养（对照组）组,观测患者营养状况、意识状态变化、应激性溃疡出血、电解质紊乱及肺部感染等.结果 重症脑出血患者术后1周时均处于负氮平衡状态,术后12d时治疗组营养指标优于对照组（P＜0.05）,治疗组早、晚期并发症均少于对照组,术后第6天应激性溃疡出血、电解质紊乱、肺部感染发生率明显低于对照组（P＜0.05）.结论 对重症脑出血患者采用早期留置胃管进行早期肠内营养支持可减少并发症,改善预后.Objective To explore the implementation of early enteral nutrition（EEN） on nutritional status and complications in postoperative patients with severe spontaneous intracerebral hemorrhage （ICH）. Methods 42 patients with severe spontaneous intracerebral hemorrhage（ GCS score ≤ 8 points） were randomly divided into the early implementation of the use of indwelling stomach tube enteral nutrition support（EEN） （treatment group） and using the traditional delayed enteral nutrition （control group） group. The nutritional status of patients, changes in state of consciousness, the hemorrhage of stress ulcer, electrolytes disorders and pulmonary infection were observed. Results Patients with severe spontaneous intracerebral hemorrhage after one week were in a state of negative nitrogen balance, twelve days after nutritional treatment group than the control group（ P 〈 0. 05 ） , the early and late complications were less than the control group, the incidence of the hemorrhage of stress ulcer, electrolyte disorders, pulmonary infection in the sixth day was significantly lower than the control group （ P 〈 0. 05 ）. Conclusion Treating Postoperative patients of severe spontaneous intracerebral hemorrhage with early indwelling stomach tube used for early enteral nutrition support can reduce complications and improve prognosis.
