郴州市实施结核病防治规划卫生经济学评价

摘　　要：目的从经济学角度评价郴州市2001—2009年实施《全国结核病防治规划(2001—2010年)》(简称《规划》)的效果与效益。方法采用成本-效用、成本-效益及成本-效果分析方法对郴州市2001—2009年结核病控制工作月报表、季报表和结核病防治投入情况及相关资料进行分析。结果 2001—2009年间共投入结核病防治经费1 611.18万元;共发现活动性肺结核患者28 041例,其中涂阳患者18 695例(治愈17 100例,治愈率为91.47%),累计减少因肺结核死亡6 844人,避免新感染133 749人,避免新发患者13 376例,共挽回163 231个DALY的损失;节约医疗费911.24万元,挽回社会总价值125 949.66万元,成本效用比为98.71,效益成本比为78.74。结论郴州市实施《规划》后实现了高发现率和高治愈率的目标,不但减轻了患者的经济负担,而且减少了大量社会劳动力的损失,取得了显著的社会效益和经济效益。Objective To evaluate the benefit and effectiveness of the implementation of the National Tuberculosis Control Planning（2001 to 2010）（NTCP） in Chenzhou from 2001 to 2009 from the perspective of the economics.Methods The methods of cost-utility,cost-benefit and cost-effectiveness analysis were adopted to analyze relevant data including monthly reports,quarterly reports,cost and benefit of tuberculosis control in Chenzhou. Results The total input in tuberculosis control was 16.111 8 million Yuan from 2001 to 2009.Total 28 041 cases with active pulmonary tuberculosis were found,of whom 18 695 cases were smear positive tuberculosis patients（17 100 cases were cured,with a cure rate of 91.47%）.The implementation of the NTCP protected 133 749 health people against the tuberculosis bacilli,and reduced 6 844 tuberculosis deaths and 13 376 new tuberculosis patients,and saved a loss of 163 231 DALYs.As a result,medical expenses of 9.112 4 million Yuan and the society total value of 1 259.496 6 million Yuan was saved.Cost-utility ratio was 98.71,and the ratio of benefit to cost was 78.74.Conclusion Through the implementation of the NTCP in Chenzhou,the goals of high case finding rate and high cure rate have been achieved.The implementation of the NTCP not only reduced the financial burden of tuberculosis patients,but also avoided large economic losses,and achieved significant social and economic benefits.
