摘  要:目的探讨左心功能不全患者血浆脑利钠肽(BNP)水平与血流动力学的相关性及其临床意义。方法入选左心功能不全患者134例,经Swan-ganz导管检查测定肺毛细血管嵌压(PCWP),干式快速免疫荧光法定量检测血浆BNP水平,超声心动图测定左室舒张末期内径(LVEDD)。结果随着心功能级别增加,血浆BNP水平、PCWP显著升高(P<0.01);直线相关回归分析表明LogBNP与PCWP呈明显正相关(r=0.515,P<0.01);LVEDD≥60 mm组BNP、PCWP明显高于LVEDD<60 mm组(P<0.01)。结论左心功能不全患者BNP与PCWP显著正相关,BNP的释放直接与心室压力负荷过度和心室容积扩张相关,BNP是反映心室受损的敏感指标。Objective To evaluate the correlation between the plasma concentration of brain natriuretic peptide(BNP)and hemodynamics and study the clinical significance of BNP in patients with decompensated left heart insufficiency.Methods BNP levels were obtained by a rapid immunofluorescence assay in 134 patients with left heart insufficiency.Left ventricular end-diastolic diameter(LVEDD)of all patients were measured by echocardiogram.Pulmonary capillary wedge pressure(PCWP)of all the patients were measured with Swan-Ganz catheters.Results BNP and PCWP levels increased with the deterioration of cardiac function(P0.01).The plasma BNP levels had a significant positive correlation with PCWP(r=0.515,P0.01).Compared of the group with LVEDD60 mm(n=71),BNP and PCWP levels were much higher than those of the group with LVEDD60 mm(n=63)(P0.01).Conclusions The plasma BNP level has an obviously positive correlation with PCWP in patients with decompenated left heart insufficiency.BNP is a sensitive index of impaired cardiac ventricle.
