单孔双管引流治疗老年人慢性硬膜下血肿的疗效观察

摘　　要：目的探讨颅骨钻单孔冲洗置双管引流治疗老年人(≥60岁)慢性硬膜下血肿的疗效。方法对2007年1月至2010年5月采用颅骨钻单孔冲洗置双管引流治疗的60例老年慢性硬膜下血肿患者的临床资料进行回顾性分析。结果治愈59例,好转1例,无手术死亡。术后并发症有血肿复发1例,钻孔处并发硬膜外血肿1例,少量气颅3例(<10 ml)。结论颅骨钻单孔冲洗置双管引流治疗老年人慢性硬膜下血肿的疗效好,损伤小,可有效减少气颅及血肿复发等并发症发生。Objective To explore the therapeutic effects of signal bone hole irrigation and set double tube drainage in treating old chronic subdural hematoma（CSDH） patients.Methods The clinical data of 60 cases patients diagnosed as old CSDH,who were treated with signal bone hole irrigation and set double tube drainage in our department from January,2007 to May,2010,were analyzed retrospectively.Results 59 cases were cured,1 case is improved,no patients died from the operation.The main complications after the Surgery included： Recurrence after hematoma（1 case）,epidural hematoma occurred in the burr hole area（1 case）,seldom pneumocephalus（3 case）.Conclusion Signal bone hole irrigation and set double tube drainage in treating old chronic subdural hematoma patients can decrease the rate of the pneumocephalus,postoperative recurrence of hematoma and other complications.Curative effect is good and the invasive is seldom.
