内镜鼻窦手术同期行鼻中隔矫正术及中下鼻甲成形术的临床观察

摘　　要：目的观察内镜下鼻窦手术同期行鼻中隔黏膜下矫正术及中下鼻甲成形术的疗效及并发症,并探讨其安全性和可行性。方法对41例慢性鼻-鼻窦炎(CRS)伴中-重度鼻中隔偏曲患者,行内镜下鼻窦开放术和同时鼻中隔矫正术及中下鼻甲成形术,并观察术后的疗效及并发症。结果术后随访6～12个月,临床治愈34例(83%),好转5例(12%),无效2例(5%),鼻中隔偏曲全部矫正;并发眼睑淤血1例,术腔不同程度粘连5例,鼻中隔血肿1例,无其他严重并发症发生。结论内镜下鼻窦手术同期行鼻中隔矫正术和中下鼻甲成形术是安全有效的,不仅节省住院时间和治疗费用还提高疗效,对预防鼻窦炎的复发及粘连有重要意义。Objective To observe complications and the therapeutic efficacy of endoscopic sinus surgery with submucous correction of nasal septum and middle-inferior turbineplasty and to explore its safety and feasibilty.Methods 41 chronic rhinosinusitis（CRS） patients with moderate-severe deviation of nasal septum received endoscopic sinus surgery.They received submucous correction of nasal septum,endoscopic sinus surgery and middle and inferior turbineplasty under local anesthesia in one-off operation.The effect and complications of the surgery were observed.Results The postoperative follow-up of 6-12 months revealed that of 41 cases,34 were clinically cured（83%）,5 were improved（12%）,and 2 were ineffective（5%）.Of the 41 cases,deviation of nasal septum was all corrected and cured,1 had complication with eyelidscongestion,5 had adherence of nasal cavity,1 had hematoma of nasal septum,no severe complications such as perforation of nasal septum were found.Conclusion Endoscopic sinus surgery with submucous correction of nasal septum and middle and inferior turbineplasty is safe and effective.It can not only reduce the hospitalization time and the therapeutic cost,but also improve the therapeutic efficacy,and it can prevent the recurrence of sinusitis and the nasal cavity adhesion.
