摘  要:目的:为了评价多平面经食管体元模型动态三维超声显像在心血管病定量诊断中的价值,我们对89个猪心、8条离体血管、16个血管支架、12个人离体二尖瓣膜、17条大、15例正常人和164例心脏病患者进行研究。图像采集采用HPSonos2500超声诊断仪和SMHz多平面经食管超声探头。三维重建采用德国TomTecECHOSCAN计算机工作站。结果:与金标准对照,多平面经食管体元模型动态三维超声显像能够准确定量左室容积(r=0.986实验;r=0.94~0.96临床)、左室游离壁心肌重量(r=0.97实验)、右室容积(r=0.95实验)、右室游离壁心肌重量(r=0.97离体;r=0.96在体)、梗死心肌的心肌重量(r=0.95)、房间隔缺损的大小(r=0.95~0.97实验;r=0.94~0.98临床)、室间隔缺损的大小(r=0.95~096实验;r=0.96~0.99临床)、心脏肿瘤的大小、二尖瓣脱垂所累及的部位,瓣叶节段和范围、二尖瓣膜的体积(r=0.98)、二尖瓣返流柱容积(r=0.85)、主动脉瓣返流柱容积(r=0.91)、血管直径(r=0.99)、血管支架直径(r=0.96)和支架与管壁的间隙。结论:多平面经合管体元模型动态三维超声显像在心血管病定量诊断中具有重要价值。To evaluate the multiplane transesophageal three - dimenstional voxel imaging (3 - D) in quantitative diagnosis of cardiovascular diseases, 89 pig heart specimens, 8 arteries, 16 vascular stents, 12 human mitral valvular specimens, 17 dogs,15 normal subjects and 164 patients with heart diseases were studied. TonecEchoscan system and HP Sonos 2500 with 5MHz multiplane transesophageal transducer was used for data acquisition and analysis respectively. hesults: ed correlation was obtained between 3 - D and golden standard: left ventricular volume (r = 0. 986 in experimental; r = 0. 94 ~ 0. 96 in patients); left vetricular myocardial mass (r= 0. 97 in experimental); right ventricular volume (r = 0. 95 in experimental); right vetricular myocardial mass (r =0. 97 in vitro; r = 0. 96 in vivo); myocardial mass of infarction (r =0. 95 in experimental); size of atriaLl septal defect (r = 0. 95 ~0. 97 in experimental; r =0. 94 ~0. 98 in patients); size of ventricular septal defect (r = 0. 95~0. 96 in experimental: r = 0. 96~0. 99 in patients); size of cardiac tumors; location, segTnent and extention of mitral prolapse; volume of mitral valves; volume of mitral regurgitant jets, volume of aortic regurgitant jets; size of arteries and stents as well as the gaps between stent and arterial wall can be quantitated accurately. Conclusions: 3 - D is a very useful technique in quantitative diagnosis of cardiovascular diseases.
