乙型肝炎患者957例血清学标志分析

摘　　要：目的　探讨乙型肝炎（ 乙肝） 病毒（ Ｈ Ｂ Ｖ） 血清学标记的表现模式。方法　共９５７ 份乙肝患者血清标本，均以 Ａｂｂｏｔｔ 多功能全自动酶免疫快速分析仪进行检测。以聚合酶链反应（ Ｐ Ｃ Ｒ） 检测 Ｈ Ｂ Ｖ Ｄ Ｎ Ａ。为表述方便，设定血清病毒学标记检测项目第１ ～５ 项的排列顺序为（１） 乙肝表面抗原（ Ｈ Ｂｓ Ａｇ） ，（２） 乙肝表面抗体（ 抗 Ｈ Ｂｓ） ，（３） 乙肝ｅ 抗原（ Ｈ Ｂｅ Ａｇ） ，（４） 乙肝ｅ 抗体（ 抗 Ｈ Ｂｅ） 和（５） 乙肝核心抗体（ 抗 Ｈ Ｂｃ） ，并以出现阳性项目的序号为该模式的代码。结果　本组血清病毒学标记模式可分为感染期模式组和恢复期模式组，两组共１６ 种模式。感染期模式组以“１４５”和“１３５”模式为主，占全部病例的４７ ％ 。恢复期模式组以“２４５”和“２５”模式为主，占全部病例的３５６ ％ 。随访结果表明，模式改变大致可分为５ 种类型。结论　本组 Ｈ Ｂ Ｖ 血清免疫学标记的模式较为复杂。除了检验误差外，产生少见模式的主要原因有低滴度抗 Ｈ Ｂｃ ，低滴度抗 Ｈ Ｂｓ 等。推测 Ｈ Ｂ Ｖ 血清免疫标志模式转换的顺序依次为ｅ 系统的转换，ｓ 系统的转换，抗 Ｈ Ｂｅ 消失或抗 Ｈ Ｂｅ 和抗 ＨObjective To analyze the results of 957 cases of HBV infection by using Abbott IMx system so as to know the current status of serological markers of HBV infection in China Methods All samples were obtained from HBV infected patients who were tested for HBsAg, anti HBs, HBeAg, anti HBe, and anti HBc by using Abbott IMx system from April 1997 to May 1998 To investigate the HBV DNA, polymerase Chain Reaction was also performed on 199 samples Results In total, 957 HBV isolates were analysed and were found that there were at least 16 different models, which can be divided in to two groups: infection model group (IMG) and recovery model group (RMG) For convenience, we set the numbers 1, 2, 3, 4 and 5 to represent HBsAg, anti HBs, HBeAg, anti HBe, and anti HBc respectively The models “145” (30 62%) and “135” (16 41%) were predominant in IMG; the models “245” (23 30%) and “25” (12 33%) were predominant model in RMG Conclusions The patients with HBV infection in China have complex manifestations in their serological markers of HBV infection Except for laboratory errors, the main causes for minority models are low titre of anti HBc or anti HBs The order of the conversion of variant models is: HBeAg/anti HBe; HBsAg/anti HBs; and clean up of anti HBe or anti HBe/anti HBs
