不同类型的岛状皮瓣在修复颜面部软组织缺损中的应用

摘　　要：目的探讨不同类型的岛状皮瓣在修复颜面部软组织缺损中的修复方法及适应证。方法鼻部皮肤缺损多采用鼻唇沟皮下组织蒂岛状皮瓣修复。鼻背部或鼻侧部较大缺损(直径在2.5～3.0cm以上)修复则采用额部滑车上动脉岛状皮瓣修复。对于下睑部、眶周、颜面部较大面积缺损的修复则应用额部颞浅动脉岛状皮瓣修复。结果 23例颜面部软组织缺损均一期岛状皮瓣修复,所有皮瓣均成活。皮瓣肤色与周围一致,瘢痕不明显。形态满意,功能恢复良好。结论根据颜面部不同部位及不同面积的缺损选择不同类型的岛状皮瓣可以获得良好的治疗效果。Objective To discuss the surgical approaches and indications of different types of island flap for repairing the facial soft tissue defects.Methods Nasolabial pedicled island flap,forehead supratrochlear artery island flap,and superficial temporal artery island flap were generally suitable for reparation of nasal defects,large defects（diameter 3.0 cm） in the dorsum and sides of nose,lower eyelid and periorbital area,respectively.Results Twenty-three patients with facial soft tissue defects underwent successful one-stage reconstruction with island flaps.The survival flaps showed normal texture,appearance and function.Conclusion Island flaps should be chosen based on the region and area of facial defect,which can achieve the satisfactory effects.
