摘 要:目的探讨自体富血小板血浆(PRP)在骨缺损部位骨组织修复面的作用。方法在新西兰大耳白兔进行自体富血小板血浆的制备,同时制造骨组织缺损创伤模型,随机分为3组:以骨组织缺损部位所加入移植材料的不同分组,A组(实验组):PRP+牛凝血酶+羟基磷灰石生物陶瓷(HA);B组(对照组):全血+牛凝血酶+HA;C组(空白组)骨缺损部位不作任何处理,直接原位周围软组织缝合。分别在术后第2、4、8 w观察局部骨组织修复情况。结果术后第2 w,3组骨缺损区未见明显成骨;术后第4 w,A组和B组少量成骨,A组优于B组,C组仅见少量纤维结缔组织增生;术后第8 w,A组和B组骨缺损基本修复,C组骨缺损骨修复欠佳,A组成骨较B组成熟。结论在促进骨组织缺损修复方面,PRP能在HA作用的基础上,进一步加速局部新骨形成、分化成熟。Objective To explore the role of autologous platelet-rich plasma(PRP)in the repair of bone defect.Methods Autologous PRP from New Zealand white rabbits were prepared,and a traumatic model of bone defect was established at the same time.Animals were randomly divided into three groups according to the material transplanted into bone defect site.Group A(experimental group)was added with PRP+bovine thrombin+hydroxyapatite(HA),group B(control group)with whole blood+bovine thrombin+HA,and group C(blank group)without any treatment except the suture of surrounding soft tissues.The progress of osteogenesis in each group was respectively observed in 2,4 and 8 w after transplantation.Results 2 w after transplantation,no obvious osteogenesis was found in the three groups.4 w later,there was a little new bone in group A and B,and group A was better than group B;only a few connective tissues were found in group C.8 w later,bone defect was almost repaired in group A and B,whereas group C displayed a poor repair.The new bone in group A was more mature than that in group B.Conclusion Based on the HA effects,PRP can further accelerate local new bone formation and maturation during the repair of bone defect.
