甲型H1N1流行性感冒轻症103例临床特征回顾分析

摘　　要：目的对甲型H1N1流行性感冒轻症患者的中医证候、证型及西医实验室检验特征进行回顾分析。方法通过观察记录103例甲型H1N1流感轻症病例的临床症状、舌苔脉象、相关实验室检查,归纳总结甲型H1H1流感轻症病例的中医证候特点、中医证型分布及实验室检查特征。结果甲型H1N1流感轻症患者,起病首发症状以发热、咳嗽、咽痛/咽痒为主,病程中常见症状体征以发热、咳嗽/咳痰、咽痛/咽痒、咽喉红肿及舌红脉数为主,实验室检查87.38%患者白细胞总数正常,但中性粒细胞分类比率升高(47.57%),淋巴细胞比率降低(44.66%),治愈出院前复查,中性粒细胞分类比率大多恢复正常,但淋巴细胞分类比率以正常(58.25%)及升高(38.83%)为主,部分患者肝功能、心肌酶、心电图异常,但无特异性症状。结论甲型H1N1流感轻症属于中医温病"温疫"范畴,初起属风热证,但同时具有"热毒"的特点,表现为发热、咳嗽、咽红、舌红等。证型以风热犯卫、热毒袭肺为主,实验室检查无特异性表现。Objective To summarize and analyze the characteristics of traditional Chinese medicine（TCM）syndromes,syndrome types and laboratory tests of mild cases infected with influenza A（H1N1）.Methods To observe clinical symptoms,TCM syndrome types and laboratory features of 103 mild cases infected with influenza A（H1N1）.Results Most of the cases began with fever,cough,pharyngodynia or pharyngealitching.In the course of the disease,the most common symptoms were fever,cough,expectoration,pharyngodynia,pharyngealitching,pharyngitis,red tongue.87.38% of patients were with normal WBC in the laboratory test while 47.57% of cases had increased proportion of neutrophils and 44.66% of them had decreased proportion of lymphocytes.After the treatment,most of the cases had nomoral proportion of neutrophils and normal or increased proportion of lymphocytes.Some cases had no related symptoms although liver function,cardiac enzymes and ECG of them were abnormal.Conclusion Influenza A（H1N1）belonged to febrile disease in the warm disease.It began with syndrome of wind-heat and had the characteristics of heat toxicity.The common symptoms were fever,cough,pharyngitis and red tongue etc.The main syndrome types were exterior syndrome of wind-heat and syndrome of heat toxicity invading lung.There were nothing specific in the laboratory tests.
