糖肾方对糖尿病肾病血流动力学易感基因表达的影响

摘　　要：目的检测3个血流动力学易感基因(AGT、AGTR2和ADRβ3)的表达量,探讨糖肾方(黄芪、生地、三七、大黄、山茱萸等)(TSF)治疗糖尿病肾病(DN)患者对易感基因表达量的影响。方法采用核素肾动态显像99mTc-DTPA检测DN患者给药前后的肾小球滤过率(GFR)。提取外周血样中总RNA,逆转录合成cDNA,实时定量聚合酶链反应(Real-TimePCR)扩增,以易感基因的绝对量/3-磷酸甘油醛脱氢酶的绝对量作为易感基因的表达量。结果 GFR水平在TSF给药前后DN患者间具有显著性差异(P<0.05)。从给药前DNⅢ期和DNⅣ期患者到给药后3个月和6个月,3个基因的表达水平均逐步提高。结论在TSF治疗作用下,GFR水平有所改善,3个基因表达量均具有不断接近正常人水平的趋势,给药6个月的基因表达改善效果优于给药3个月,揭示通过TSF长期调理逐步改善DN患者体内代谢紊乱,缓解其血流动力学异常状况。AIM To explore the effect of Tangshen Formula（TSF）（Astragali Radix,Rehmanniae Radix,Notoginseng Radix et Rhizoma,Rhei Radix et Rhizoma,Evodiae Fructus,etc.） on hemodynamics related to susceptibility alleles’expression of AGT,AGTR2 and ADRβ3 in patients with diabetic nephropathy（DN）.METHODS Glomerular filtration rate（GFR） was detected by nuclide renal dynamic imaging with 99mTc-DTPA in DN patients before and after the administration of TSF.Total RNA was extracted from peripheral blood sample,and cDNA was synthesized by reverse transcription.Gene amplification was detected by Real-Time PCR method,the absolute amount of susceptibility alleles versus that of GAPDH was considered as the relative quantization of the expression level of susceptibility alleles.RESULTS There were significant different（P 0.05） GFR levels between DN patients before and after the administration of TSF.From the DNⅢ and DNⅣ patients to the DNⅣ patients after administration of TSF for 3 months and 6 months,three genes expression level were gradually increased.CONCLUSION Under the treatment of the TSF,GFR level has improved,all of the three allelic genes expression are continuously close to normal levels.Improvement in allelic gene expression of DNⅣ at the end of six months by TSF is better than that at the end of three months,suggesting that DN patients are gradually improving metabolic disorders and alleviating the abnormal hemodynamic status by long-term conditioning by TSF.
