摘  要:目的了解日本血吸虫病疫区人群外周血单核细胞在一组特异性的重组基因候选疫苗分子刺激下的增生反应。方法选择洞庭湖日本血吸虫病洲岛型疫区两个岛(青山和严家山)117名居民(实验组)和来自非疫区的15名居民(对照组)作为研究对象,观察其外周血单核细胞在体外培养的条件下用日本血吸虫5种疫苗候选抗原的重组基因分子及可溶性成虫抗原刺激时的增生反应,以评价这些候选抗原对细胞的刺激效应。结果其中副肌球蛋白和28kDa谷胱甘肽-S-转移酶(GST)分别对38.5%和42.5%的研究对象有增殖效应(刺激指数≥3.0),与可溶性成虫抗原的刺激水平(51.3%)相当。其它3种重组抗原(脂肪酸结合蛋白、22kDa表膜相关蛋白和3-磷酸甘油醛脱氨酶)仅对3%~8%的对象有刺激作用。严家山和青山岛上分别有90.0%和42.1%的对象的单核细胞对成虫抗原有增殖应答,其差异极显著(X2=16.88,P<0.01);分别有77.3%和34.7%对象的单核细胞对GST存在增殖应答(X2=13.9,P<0.05);对副肌球蛋白和脂肪酸结合蛋白亦存在类似倾向,但无显著性差异。结论重组基因抗原即副肌球蛋白和GST28能诱导疫区人群中38.0/~42.0%的个体产生外周血单核细胞(PBMC)增殖反应,其频率与成虫抗原诱导的相当。来自不同?Objective To explore antigen-stimulated proliferation of peripheral bloodmononuclear cells (PBMC), which were from people living in the area endemic for schistosomiasisjaponica, against a panel of recombinant S. japonicum antigens. Methods According to the resultsof epidemiological survey, 117 individuals (subjects) living on two islands (Qingshan and Yan-jiashan) in Dongting Lake area endemic for schistosomiasis japonica and 15 control individuals froma non-endemic area were sampled. Peripheral blood mononuclear cells from these people were as-sessed in order to observe antigen-stimulated proliferation against a panel of antigens including fiverecombinant S. japonicum antigens and S. japonicum soluble adult worm antigen (AWA). ResultsTwo recombinant antigens, paramyosin and 28 kDa glutathione-S-transferase, stimulated PBMCproliferation (Stimulation index≥3.0) in 38.5% and 42.5% of subjects respectively, a level similarto that induced by AWA (51.3%); three other recombinant antigens-a fatty acid binding protein,22 kDa tegumental membrane associated antigen and glyceraldehyde-3-phosphate dehydrogenase-stimulated PBMC proliferation in only 3.0%~8.0% of subjects. There is significant difference inAWA-stimulated PBMC proliferation between the subjects from Yanjiashan island (90.0%) andthose from Qingshan island (42.1% ) (X2=16.88,P<0.01). Moreover, a highly significant differ-ence was observed for glutathione-S-transferase which stimulated proliferation in 77.3% of subjectsfrom Yanjiashan compared with only 34.7% of subjects from Qingshan (X2=13.09, P<0.05). Asimilar, but not significant,trend was observed for paramyosin and the fatty-acid binding protein.Conclusious Two recombinant antigens-paramyosin and glutathione-S-transferase-can inducethe PBMC proliferation of 38.0%~42.0% subjects from area endemic for schistosomiasis japonica,the frequencies similar to those induced by AWA, and there are markedly high differences in PBMCproliferation, which were stimulated by specific antigens, between individuals f
