核医学甲状腺系列检查在甲状腺炎诊断中的价值

摘　　要：目的:探讨甲状腺吸碘率、甲状腺显像定量分析及甲状腺激素检测等核医学甲状腺系列检查在甲状腺炎的早期诊断价值.方法:对33例亚急性甲状腺炎患者及17例正常对照者进行甲状腺吸碘率及甲状腺激素检测,同时应用SPECT进行甲状腺显像及定量分析.结果:甲状腺炎患者的3小时和24小时的吸碘率均显著低于正常对照组,而3/小时和24/小时的吸碘率比值则显著高于正常组,p<0.01,甲状腺比值明显低于正常组,p<0.01,但甲状腺炎组总T_3、T_4均值与正常组比较无显著差异,TSH则显著降低,P<0.01.结论:在甲状腺炎的诊断价值中,甲状腺比值判断甲状腺摄碘功能具有吸碘率测定同样价值,并均优于甲状腺激素测定,但甲状腺显像可观察甲状腺形态、结构变化.Purpose:To study the diagnostic value of iodic absorptivity of thyroid gland, ratio of thyroid to sal-vary, and thyroid hormone measure in the thyroidits patients. Methods: 33 patients with thyroiditis and 17 normal controls have been examined the iodic absorptivity of thyroid gland and thyroid hormone, and performed the thyriod scan by SPECT, then calculated the ratio of thyroid to salvary gland. Results: Iodic absorptivity at 3 hour and 24 hour, ratio of thyroid to salivary gland in patients with thyroiditis are notably lower than those in controls,P<0.01 ,and the ratio of 3 hour to 24 hour iodic absorptivity in patiets is higher than that in controls. p < 0.01.There is no difference between the total T3 and T4 of thyroid gland in patient and that in normals, but the TSH in patients is smaller than that in controls; p<0.01 . Conclusions:Thyroid scan may observe the shape and construcion of thyroid gland, and the thyroid ratio show the same diagnostic value as the examnation of iodic absorptivity. The diagnostic value of the thyroid scan and measure of iodic absorptivity is better than that of the measure of thyroid hormone.
