股骨近端髓内钉与动力髋部螺钉治疗股骨粗隆间骨折的比较

摘　　要：目的：探讨应用股骨近端髓内钉（PFN）与动力髋螺钉（DHS）治疗股骨粗隆间骨折的临床效果。方法：自2000年1月～2004年1月，治疗股骨粗隆间骨折患者116例，其中应用DHS60例饵组），PFN56例（B组），比较手术时间、术中输血量、血红蛋白变化量、术后引流量、住院天数、骨折愈合时间以及髋关节功能评分，并行统计学处理。结果：两组病人均获得12—24月的随访，所有骨折均愈合，无不愈合病例及死亡病例。两组患者的术中输血量、住院天数、骨折愈合时间以及髋关节功能Harris评分均基本相似（P〉0．05），但是PNF组在手术时间、血红蛋白变化量及术后引流量均低于DHS组（P〈0．05）。结论：使用PFN和DHS治疗股骨粗隆间骨的结果相似。但是PFN组有手术时间短、失血量少、切口小等优点。Objective： To discuss the clinical results of proximal femoral nail （PFN）and dynamic hip screws （DHS）in treatment of intertroehanteric fracture of femur. Methods： From January 2000 to January 2004, 116 patients with intertroehanteric fracture of femur were divided into two groups, 60 of them were treated with DHS （group A） and the rest 56 cases were treated with PFN （group B）. We analyzed and compared the time of operation, amount of blood transfusion, amount of hemoglobin variance, amount of post-op drainage, hospitalization days, fracture healing time and hip joint function sore. All the data of the two groups were observed and analyzed statistically. Results： All the patients were followed up for 12 to 24 months, the fractures healed in all cases and no one was dead. There was no marked difference in amount of blood transfusion, hospitalization days, fracture healing time and the mean postoperative Harris score between the two groups （P〈0.05）. But the operation time, amount of hemoglobin decreasing and postoperative drainage in PFN groups decreased significantly when compared with DHS groups （P〉0.05）. Conclusion： The clinical results of proximal femoral nail （PFN） and dynamic hip screws （DHS） in treatment of intertrochanteric fracture of femur are similar, but advantages of PFN group in treating intertrochanteric fracture included short operation time, little exposure and less amount of blood loss.
