慢性肾衰竭合并肺部感染65例分析

摘　　要：目的探讨慢性肾衰竭患者合并肺部感染的发生率、发病诱因、治疗方式效果、预后及转归,为预防和治疗慢性肾衰竭并肺部感染提供依据.方法对昆明医学院附属延安医院肾脏内科2007年1月至2009年12月确诊为慢性肾衰竭合并肺部感染的65例患者的病例资料进行回顾性分析,探讨慢性肾衰竭患者合并肺部感染的诊治情况.结果 2007年1月至2009年12月该科住院诊断为慢性肾衰竭病例(资料完整)患者295例(其中血液透析150例,腹膜透析18例,内科药物治疗125例,肾移植2例),其中出现肺部感染65例,比例为22%,男性31例,女性34例,年龄(13～88)岁,平均(62.5±11.2)岁,治疗后好转和痊愈48例(占73.8%),自动出院5例(7.7%),死亡12例(18.5%).结论慢性肾衰竭患者由于多种原因极易发生肺部感染,特别是重症肺炎,死亡率明显增加,因此防治引起肺部感染的诱因、及时有效的抗菌治疗及呼吸辅助治疗尤为重要.Objective To investigate the inducement, incidence, treatment, outcome and prognosis of patients with chronic renal failure and pulmonary infection simultaneously, and provide envidences for the treatment of those patients. Methods We retrospectively analyzed the clinical data of 65 patients with chronic renal failure with pulmonary infection from January 2007 to December 2009 in our department. The diagnosis and treatment of patients with chronic renal failure and pulmonary infection was analyzed. Results During January 2007 to December 2009, we accepted 295 patients with chronic renal failure. Among the 295 patients, 150 patients accepted hemodialysis, 18 patients underwent peritoneal dialysis, 125 patients accepted medical therapy, and 2 cases experienced renal transplantation. Pulmonary infection was found in 65 patients （22%） , including 31 male and 34 female paitents, aged from 13 to 88 years, the mean age was 62.5± 11.2 years. After treatment, 48 patients were cured （73.8%） , 5 patients refused further medical treatment and were discharged （7.7%）, 12 patients died （18.5%）. Conclusion The Chronic renal failure patients as a result of a variety of reasons are prone to develop pneumonia, especially severe pneumonia. And the death rate was very high. Thus, it is of great important to deal with the risk factors of pneumonia and effective antibiotic and auxiliary treatment werealso very necessary once it occurred.
