摘 要:目的:①检测心力衰竭(心衰)患者血浆中IC53基因自身抗体水平;②寻找IC53自身抗体与心功能、不同病因等因素的相关性。方法:①观察组:188例,为山东莱芜市人民医院2006-06-2008-06符合心衰诊断标准的患者;对照组:160例,为在莱芜市人民医院行健康体检的健康人。②所有入选者均排除内分泌性、自身免疫性及感染性疾病,采集血样前3d避免服用免疫抑制剂、β-受体阻滞剂、血管紧张素转化酶抑制剂,入选后予心脏X线检查和超声心动图检查。③以合成肽作为抗原,首先应用酶联免疫吸附测定技术,检测348例受试者血浆中IC53自身抗体;随后选取实验结果阳性的标本为样品侧,BSA为对照侧,用Wester blot检测血浆中的抗体表达。结果:①心衰组IC53自身抗体的阳性率为21.3%(40/188),明显高于对照组5%(8/160);不同病因患者IC53自身抗体的阳性率差异无显著性;心功能Ⅳ级的阳性率明显高于心功能Ⅲ、Ⅱ级的阳性率。②IC53自身抗体在相同心功能分级不同性别之间的阳性率有显著性差异,女性明显高于男性。③Western blot结果:样品侧和对照侧蛋白表达有差异。结论:IC53自身抗体存在于多种心脏病患者的血浆中,可能与心衰时心肌结构变化和功能下降有关,与心脏病病因无明显因果关系。Objective:To explore whether antibody against IC53 gene is related to CHF induced by different cardiac diseases.Method:①188 congestive heart failure patients,who underwent therapy from June 2006 to June 2008 in Laiwu People's Hospital,and 166 healthy subjects,which are chosen as controls from clinical healthy examination population,are both studied randomly.Those subjects with endocrine disorders,autoimmune diseases or injections are excluded.②Based on synthetic peptides as antigens,enzyme-linked immunosorbent technique was used firstly to detect Antibody against IC53 and Wester blot was adopt later to screen the serum antibody level from positive patients with Antibody against IC53 and controls.Result:①The positive rates of serum for IC53 genewere 21.3% and 5% in CHF patients and controls respectively;②the frequencies of the antibody in patients with cardiac function of class Ⅳ was signicantly higher than those with cardiac function of class(Ⅲ,Ⅱ);③the frequencies of the antibodies in female patients was higher than that in males with the same class of cardiac function.The result of Wester blot showed that the expression of IC53 was significantly different between patients and controls.Conclusion:It is indicated that autoantibody against IC53 gene can be detected in patients with CHF.It is suggested that autoantibody against IC53 gene may be related to LV remodeling and/or dysfunction in CHF.
