摘 要:目的掌握PCA(PolyCheck AllergyAssay系统)和敏筛(AllergyScreen系统)两种血清特异性IgE(sIgE)检测方法,在变应原检测及变应性鼻炎(AR)诊断中的客观性和异同。方法于2007年4月,按随机多级抽样和整群抽样方法,对抽取的206例河北省沧州市盘古村民,再随机分组138例做PCA检测,68例做敏筛检测。同时对2007年4至10月到我院就诊的280例自愿选择sIgE检测,有AR病史患者按诊号分组:单号108例做PCA检测;双号140例做敏筛检测。并将其结果与322例自愿选择SPT(skin prink test)检测的患者比较。结果①盘古村村民变应原sIgE检测,PCA组阳性40例(28.89%),敏筛组阳性13例(19.12%)。经检验2组阳性预测值之间无统计学差异(χ2=2.32,P>0.05);再比较有AR病史患者,其PCA和敏筛与SPT检测阳性预测值分别为71.30%、45.00%和64.91%,多样本率的检验χ2=21.98,P<0.01。两两比较仅PCA与SPT之间的无统计学差异(P>0.05)。②变应原分类检出率,敏筛对食物类较灵敏,PCA对动物上皮类更灵敏。③盘古村民PCA组AR诊断阳性率7.97%(11/138),敏筛组为7.36%(5/68)。经检验两组之间无统计学差异(χ2=0.56,P>0.05)。结论 PCA和敏筛2种方法均有可靠性、安全性和特异性,易操做、敏感度高且各有所长,明确变应原对预防和治疗变应性疾病有重要意义,是变态反应临床和研究不可缺少的客观指标。This study aimed to evaluate the similarities and differences of AllergyScreen(AS) test and PolyCheck Allergy(PCA) test in quantitative detection of serum allergen-specific IgE antibodies(sIgE).In April 2007,206 cases were collected from the villagers of Pangu Village,Cangzhou City,Hebei Province,according to multi-stage random sampling and cluster sampling method.138 cases were tested with PCA,while 68 cases with AS.At the same time 280 cases of outpatients with AR who accepted sIgE testing voluntarily were divided into two groups according to patient number: 108 cases of odd numbers test with PCA and 140 cases of double numbers with AllergyScreen.And the results were compared with that of 322 cases detected with skin prink test(SPT).As results,sIgE positive cases of villagers of Pangu were 40 cases(28.89%) in PCA group and 13 cases(19.12%) in AllergyScreen group.Positive predictive value between the two groups are no statistically significant differences(χ2= 2.32,P〉0.05).The positive predictive value of PCA,AllergyScreen and SPT in detecting cases with AR history were 71.30%,45.00% and 64.91% respectively,and multiple sample rates tests shows χ2= 21.98,P〈0.01.There was no statistical significance difference between PCA and SPT(P〉0.05).As to allergen classifying detection rate,AllergyScreen was more sensitive in food,while PCA was more sensitive in animal epithelium.We also found that the positive rate of AR diagnosis was 7.97%(11/138) in PCA group and 7.36%(5/68) in AllergyScreen group,but there are no statistical difference between the two groups(χ2= 0.56,P〉0.05).Our study demonstrated that PCA and AllergyScreen both are reliable,secure,and specific in clarifying allergens,which is important on prevention and treatment of allergic disease.
