摘 要:目的探讨妊娠糖尿病(GDM)患者胰岛β细胞功能与氧化应激的关系。方法入选2008年1月-2009年6月GDM患者56例(GDM组)、正常妊娠妇女60例(正常妊娠组)和正常育龄妇女50例(对照组)为受试者,测定3组的血糖、血脂、胰岛素等生化指标和血浆丙二醛(MDA)、总抗氧化能力(TAOC)、红细胞谷胱甘肽(GSH)和超氧化物歧化酶(SOD)水平。用稳态模型胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMAβ)和胰岛素敏感指数(ISI)评价胰岛β细胞功能。结果与对照组和正常妊娠组比较,GDM组的MDA水平升高,TAOC、GSH和SOD水平下降(P均<0.05),空腹血糖(FBG)、空腹胰岛素(FINS)和HOMA-IR升高,HOMAβ和ISI下降(P均<0.05)。Logistic回归分析显示,影响胰岛β细胞功能的因素有:年龄、孕期、体质量指数、FBG、FINS、MDA、TAOC、GSH和SOD(P均<0.05或0.01)。结论妊娠糖尿病患者胰岛β细胞功能的受损可能与氧化应激有关。Objective To study the relationship between pancreatic β cell function and oxidative stress in gestational diabetes mellitus(GDM) patients.Methods GDM patients(GDM group,n=56),normal pregnant women(normal pregnant group,n=60) and healthy women(control group,n=50) were enrolled from January 2008 to June 2009.Serum glucose,lipids,insulin,plasma malondialdehyde(MDA),total antioxidation capability(TAOC),superoxide dismutase(SOD) and erythrocyte glutathione(GSH) were measured among the groups.Insulin resistance index(HOMA-IR),insulin secretion index(HOMA β) and insulin sensitivity index(ISI) were calculated.Results Compared with the normal pregnant group and control group,the GDM group reported increased level of MDA,decreased levels of TAOC,GSH and SOD,higher FBG,FINS and HOMA-IR,lower HOMA β and ISI(all P〈0.05).Logistic regression analysis showed that age,duration of pregnancy,BMI,FBG,FINS,MDA,TAOC,GSH,SOD and impaired pancreatic β cells function of GDM patients had significant correlation(P〈0.05 or 0.01).Conclusion Impaired pancreatic β cells function of GDM patients may be related to oxidative stress.
