摘  要:目的:寻找系统性红斑狼疮病例中的关键指标,为诊断提供参考依据。方法:采集350例系统性红斑狼疮患者和85例对照组的资料,运用SPSS软件进行主成分分析,评价各个指标对疾病影响的严重程度,并通过绘制ROC曲线来比较不同诊断指标在多个诊断界值条件下对应的灵敏度-特异度曲线的差异,根据曲线下面积评估各个指标的诊断准确度。结果:根据主成分分析,第一公因子方差贡献率最大,为31.9%(旋转前),第一公因子中载荷值较大的指标分别为C4、ds-DNA、C3和ANA。根据ROC曲线,ds-DNA、C3、C4和ANA的曲线下面积分别为0.87、0.82、0.74、0.73。结论:ds-DNA、C3、C4和ANA等四个指标相对于其余临床指标,与系统性红斑狼疮疾病有较大的关系,对早期发现和诊断系统性红斑狼疮有一定的指导意义。Objective: To search for the major indicatrixes to assist diagnosis of the systemic lupus erythematosus.Methods: Three hundred and fifty systemic lupus erythematosus patients were selected,and were compared with the 85 controls.the SPSS software was used to run the principal component analysis and ROC curve analysis to compare the severity of every indicatrixes through the extracted common factors and get the specificity-sensitivity curve differences for every indicatrix under the diagnostic cutoff conditions through different clinical experiments.Results: The variance contribution of F1(first common factor)was the maximum,accounting for 31.9%(before rotation)and the much larger indicatrixes of F1 were C4,ds-DNA,C3 and ANA.The area under curve of ANA,ds-DNA,C3 and C4 were 0.87,0.82,0.74 and 0.73,respectively according to the ROC curve.Conclusion: The four indicatrixes of ANA,s-NA,C3 and C4 are indicated to be more important than the other indicatrixes for he early detection and diagnosis of SLE.
