台州医院2007～2010年产前超声检查胎儿异常结果分析

摘　　要：目的:研究产前超声检查对胎儿结构异常的诊断价值,分析漏诊原因及台州地区胎儿畸形状况。方法:回顾性分析台州医院2007年1月～2010年6月52 760例中晚期孕妇的产前超声检查图片。结果:52 760例中,单胎51 687例(97.97%),双胎1 073例(2.03%),共检出胎儿畸形1 245例,检出率为2.36%。1 245例畸形胎儿中,单发畸形1 014例,占81.45%,多发畸形231例,占18.55%。1 245例经引产、尸检及出生后证实,超声漏诊72例,漏诊率5.78%。结论:台州地区胎儿畸形发生率高于国外的平均值,且胎儿畸形种类繁多复杂。产前超声检查采用顺序部位方法进行规范化的检查,胎儿畸形的检出率高。超声检查是诊断胎儿畸形的重要手段,是产前诊断的首选方法,但对于一些小畸形及单纯腭裂、肛门闭锁、生殖系统畸形等漏诊率极高,应引起高度的重视。Objective：To explore the diagnostic value of prenatal ultrasonography to fetal structural abnormalities,and to analyze the reasons of missed diagnosis and the situations of fetal malformation in Taizhou.Methods：52 760 late pregnancy women who underwent fetal ultrasound screening in Taizhou hospital from January 2007 to June 2010 were analyzed retrospectively.Results：Among 52 760 cases,51 687 cases of single birth（97.97%）,and 1 073 cases of twins（2.03%）.1245 cases of fetal malformations were checked out,the detection rate was 2.36%,of which 1014 cases with single abnormality（81.45%） and 231 cases with multiple malformations（18.55%）;1 245 cases were confirmed by induced labour,autopsy and after birth confirmation;72 cases missed diagnosis（5.78%）.Conclusion：The incidence of fetal malformations in Taizhou is higher than the foreign average,with a wider range and more complicated fetal malformations.Prenatal ultrasonography must apply a method of examining different parts in a certain order with standardized examination.The high detection rate of fetal malformations is an important means of fetal malformation diagnosis and is the first choice of prenatal diagnosis.But for some small defects and single cleft palate,atresia and reproductive system abnormalities,the rate of missed diagnosis is very high,which implicates that particular attention should be paid.
