子宫动脉栓塞术治疗产后大出血18例临床观察

摘　　要：目的探讨子宫动脉栓塞(UAE)手术治疗产后大出血的临床应用价值。方法 18例产后大出血患者行子宫动脉栓塞(UAE)手术,超选择插管进入双侧子宫动脉及侧支供血血管,经造影证实后,注入明胶海绵颗粒予以栓塞,重复造影,证实栓塞成功。结果 18例患者共栓塞子宫动脉35支,侧支供血血管共3支,栓塞成功率100%,3例患者出血逐渐停止,15例患者术后即时止血,无一例出血复发,无严重并发症发生。结论 UAE手术治疗产后大出血即时、微创、恢复快、并发症少、疗效高,保留了子宫、卵巢正常的生理功能,有利于患者的健康,提高患者的生活质量。Objective To evaluate the clinical application value of uterine arterial embolization for postpartum hemorrhage.Methods Eighteen patients with postpartum hemorrhage adopted the uterine arterial embolization treatment,they were chosen to be catheterized of both bilateral uterine arteries and lateral veins using supper-selection technique.Gelfoam was injected into arteries for embolization after angiography proved,then followed by another angiography to confirm the success of embolization.Results Embolization of 35 uterine arteries and thirteen lateral veins of all 3 case were successfully carried out.Fifteen cases of postpartum were controlled immediately and others were gradually controlled.No setback and no serious complication.Conclusion Uterine arterial embolization is a treatment that is instant,precise,minimal wound,quick recovery,with scarce complications.And the treatment can save the physiological function of uterine and ovary,in favor of patients health,and it has a superior clinical applied value.
