摘 要:目的:探讨适合早产极低出生体重儿的喂养方式及其对早产极低体重儿喂养不耐受及早期生长发育的影响。方法:将在NICU住院的60例早产极低出生体重儿随机分为两组,对照组30例采用常规胃管喂养法;治疗组30例在对照组的常规治疗基础上,出生后24 h内开始微量喂养,每天的奶量用微量输液泵通过胃管持续间断注入,喂奶时间为2 h喂1次,滴入2h后暂停2 h,当VLBWI每次奶量增加至15~20 ml/kg体重时将2 h喂养1次改为3 h喂1次。两组喂养时间均直至患儿有吸吮和吞咽能力、能经口喂养后停止鼻饲。比较并记录两组:①每次喂奶量及发生喂养不耐受的例数。②生长发育指标。结果:给予早产极低体重儿早期间歇持续微量喂养组的早产极低体重儿过渡到经口喂养时间快,胃肠喂养不耐受发生率明显减少,促进早产儿早期生长发育。结论:早期间歇持续微量喂养是VLBWI简便而有效的辅助喂养方法,对VLBWI的早期生长发育有重要意义。Objective: To explore the feeding patterns for very low birth weight infant ( VLBWI), and its impact on feeding intoler- ance and early growth in VLBWI. Methods: 60 VLBWI in NICU were divided into two groups. Control group (30 cases) received the method of conventional gastric tube feeding; Treatment group (30 cases), based on conventional treatment, started feeding within 24 h after birth, and the daily milk through the tube with a micro infusion pump intermittent continuous injected to VLBWI per 2 hours, suspended 2 h after 2 h. When the milk weight increased to 15 -20 ml/kg, feeding 1 time per 3 hours. Both group stopped nasal feeding until VLBWI could suck and swallow by themselves. Finally, the amount of each feeding, the number of feeding intolerance and the indicators of growth and development were recorded and compared in two groups. Results: VLBWI given intermittently continuous micro feeding early in treatment group transition to oral feeding earlier, significantly reduced the incidence of feeding intolerance, and promoted early growth and development. Conclusion: Early intermittent continuous feedings is a simple and effective method of complementary feeding to VLBWI, it may play an important role in the early growth of VLBWI
