家庭无创机械通气治疗慢性阻塞性肺病并发呼吸衰竭30例疗效观察

摘　　要：目的:探讨家用无创机械通气对慢性阻塞性肺疾病并发呼吸衰竭患者的治疗效果。方法:对30例经过医院内正规治疗的慢性阻塞性肺疾病并发呼吸衰竭患者在其病情稳定后,应用家用BiPAP无创呼吸机进行治疗,在医院内调试、应用48h,评价其一般情况、动脉血气稳定后,让其出院在家中进行无创机械通气,定期随访观察治疗效果。结果:家庭无创机械通气前及治疗6个月后,呼吸困难程度分级分别为4和3级;肺功能检测结果(FEV1)分别为(29±14)%和(35±9)%;动脉血气PO2为53.1±5.9mmHg和61.0±7.0mmHg,PCO2分别为55.2±5.2mmHg和45.8±5.9mmHg;住院天数分别为21±9d和12±5d。结论:家庭无创机械通气治疗慢性阻塞性肺疾病并发呼吸衰竭患者疗效可靠,且较安全。Objective： To explore the effects of home noninvasive mechanical ventilation（NIV） on chronic hypercapnic respiratory failure in patients with COPD.Methods： To study the effects due to COPD before and after 6 months the initiation home ventilation we tested the dyspnea grade,arterial PO2 and PCO2,forced expiratory volume in one second of pred （FEV1）,hospital admission and so on.Results： The treatment with NIV was good and the median of machine usage was 6.6±3.9h per day.PCO2 decreased from 55.2±5.2 to 45.8±5.9 mmHg,and PO2 increased from 53.1±5.9 to 61±7.0 mmHg. FEV1 % pred increased from 29±14 to 35±9.Hospital admission decreased from 21±9 to 12±5 days.Conclusion： The mean effect of home NIV in patients with COPD is good at gas exchange,pulmonary function,exercise tolerance and life quality.To tell more data we need more cases and longer time.
