摘 要:目的研究细胞色素P450酶2C9(CYP2C9)基因多态性对华法林用药剂量的影响,指导临床用药。方法纳入257例华法林抗凝稳定治疗的患者,以Realtime-PCR法检测CYP2C9*3的基因型,将受试者分为CYP2C9*1/*1、*1/*3、*3/*3共3组,统计分析3组受试者华法林用药剂量,确定患者基因型对华法林剂量的影响。结果在257例受试者中野生型个体(*1/*1)232例,剂量为(4.06±1.72)mg/d;杂合型突变体(*1/*3)20例,剂量为(2.70±1.36)mg/d;纯合突变体(*3/*3)5例,剂量为(0.56±0.25)mg/d。野生型个体服用标准剂量,杂合型突变体用量降低了40%,纯合型突变体用量降低了86%。结论 CYP2C9基因多态性对华法林用量有显著的影响。Objective To study the effect of cytochrome p450 2C9 (CYP2C9) gene polymorphism on the initial dose of warfarin in order to guide clinical medication. Methods 257 patients who received anticoagulant therapy with warfarin underwent realtime - PCR detection of CYP2C9 genotype. They were divided into three groups, CYP2C9 * 1/* 1 , * 1/* 3, and * 3/* 3. Statistical analysis of their initial doses of warfarin was used to determine the effect of the genotype of patients on the initial measurement of warfarin. Results Of the 257 subjects, 232 showed wild genotype ( * 1/ * 1 ) at the dose of (4.06 ±1.72) mg/d, 20 beterozygous mutants ( * 1/* 3 ) at the dose of(2.70 ± 1.36) mg/d and 5 homozygous mutants ( * 3/* 3) at the dose of(0.56 ± 0.25 ) mg/d. The wild - type individuals took the standard dose, heterozygous mutant individuals consumed a dose that decreased by 40% , and homozygous mutant individuals were given a dose that was reduced by 86%. Conculsions The gene polymorphism of CYP2C9 has a significant impact on the initial dosage of warfarin in China.
