三维彩色直方图血流定量分析在乳腺肿瘤诊断中的应用价值

摘　　要：目的探讨三维彩色直方图血流定量分析参数在乳腺良恶肿瘤的鉴别诊断中的应用价值。方法应用GE Voluson 730三维彩色超声仪对111例患者133个乳腺肿块进行彩色多普勒检查,其中恶性肿瘤36个,良性肿瘤97个,所有病例均经手术病理及随访复查证实。采用三维彩色直方图技术测量肿瘤的体积(volume,V)、血管指数(vas-cularization index,VI)、血流指数(flow index,FI)和血管血流指数(vascularizatiowflow index,VFI)。结果恶性肿瘤组的V值、VI值、FI值和VFI值均显著高于良性肿瘤组(P均<0.05)。V,VI,FI,VFI对乳腺良恶肿瘤的鉴别诊断均为有效,各指标据ROC曲线确定的诊断截值均具有较高的敏感性和特异性,其中,V指标诊断乳腺良恶肿瘤的价值最高,VI,VFI次之,FI较差。结论三维彩色直方图血流定量分析参数在鉴别诊断乳腺良恶性肿瘤中有较高的应用价值,可以作为鉴别乳腺良恶性肿瘤的定量分析依据。Objective To investigate the diagnostic value of quantitative analysis of blood flow using three-dimensional color histogram in the patients with breast tumors. Methods 111 patients with 133 breast lumps, including 97 benign tumors and 36 malignant tumors which were further confirmed by pathological examination or followed-up, underwent Color Doppler ultrasonographic examination by GE Voluson 730 ultrasonic instrument. The volume （V）, vascularization index （VI）, flow index （FI） and vascularization-flow index （VFI） were measured in all breast tumors by three-dimensional color histogram. Results The indices of V, VI, FI and VFI in malignant tumors group were significantly higher than those in benign tumors group（all P〈0.05）. The indices of V, VI, FI and VFI were all valid for differential diagnosis between benign and malignant breast tumors, and demonstrated high sensitivity and specificity at each of their diagnostic cut-off values by ROC curve. In these indices, V was the best index for the diagnosis of breast tumors, VI and VFI were the second, and FI ranked last. Conclusions The quantitative analysis indices of blood flow using three-dimensional color histogram plays an important role in the diagnosis of breast tumors and could be used as quantitative analysis means for differential diagnosis between benign and malignant breast tumors.
