半月板隐匿性损伤25例分析

摘　　要：目的探讨半月板隐匿性损伤的发生机制、临床表现、诊断和关节镜治疗。方法对25例半月板隐匿性损伤行关节镜手术治疗,就漏诊误诊的原因、临床特点及疗效进行回顾性总结。结果隐匿性半月板损伤多为半月板层裂、桶柄裂、硬结样变性,部分伴有半月板囊肿,关节镜下外观表现近似于正常半月板,极易漏诊、误诊,行关节镜诊治后症状消失。结论半月板隐匿性损伤有其独特的临床特点,依据分型可掌握诊断及治疗要点。术前详细查体、准确阅片,术中仔细探查、积累镜下手术经验,有助于提高关节镜手术疗效,减少漏诊、误诊。Abstract Objective To explore the mechamsm, chnical charactenstics, diagnosis and arthroscopic treatment ol occult meniscal injury. Methods Twenty five cases of occult meniscal injury received arthroseopic surgery, the cause of misdiagnosis, the clinical characteristics of occult meniscal injury and the curative effect of arthroscopic treatment were analyzed.Results The occult meniscal injury varied, which included horizontal tear, bucket-handle tear, degenerated scleroma. Eight patients with horizontal tear were associated with meniscus cyst. Arthroscopic appearance was similar to normal meniscus and easily missed. Alter experienced arthroseopic treatment the symptoms disap peared.Conclusion The occult meniscal injury has its own unique clinical characteristics. Preoperative detailed checkup, accurate reading films, careful intraoperafive exploration and accumulated experience in arthroscopic surgery would do help to improve efficacy of arthroseopie surgery and reduce misdiagnosis.
