器质性心脏病并阵发性室性心动过速临床分析

摘　　要：目的探讨器质性心脏病并发阵发性室性心动过速(paroxysmalventriculartachycardia,PVT)相关因素。方法 200例患者进行心电图、Holter、彩色多普勒超声心动图、冠状动脉造影、电解质等检查及心功能评级,分析患者心脏病变及心律失常情况。结果 200例患者中,冠心病77例,高血压性心脏病52例,心瓣膜病40例,扩张型心肌病31例。其中发生PVT的疾病构成:冠心病47例(43.5%),高血压性心脏病26例(24%),心瓣膜病21例(19.4%),扩张型心肌病14例(13%)。冠心病发生率明显高于其他病种。左室舒张期末内径(Left Ventriculor end diameters dimension,LVEDd)和左室肥厚(Left Ventriculor hypeitrophy,LVH),PVT组与非PVT组比较差异有统计学意义,P<0.01。在PVT患者中,心功能≥Ⅲ级多于心功能≤Ⅱ级,P<0.01,血钾≤3.5 mmol/L病例数比血钾≥3.5 mmol/L患者多,P<0.05。结论器质性心脏病并PVT与患者LVEDd、LVH、心功能级别和低血钾相关。Objective To investigate the correlative factors of organic heart disease with paroxysmal ventricular tachycardia（PVT）.Methods Two hundred subjects were enrolled in this research.With the intent to evaluate the heart function and investigate the relationship between heart abnormalities and arrhythmia,all of the participants accepted the examinations,including electrocardiogram,dynamic electrocardiogram,Doppler echocardiography and electrolyte.Results There were 47（43.5%） individuals complicated with PVT in 77 subjects with coronary artery disease,26（24%） complicated with PVT in 52 individuals with hypertensive heart diseases,21（19.4%） PVT in 40 subjects with valvular disease,and 14（13%）PVT in 31 subjects with dilated cardiomyopathy.All of the subjects who suffered from organic heart disease had the abnormalities of LVEDD and LVH.The value of LVEDD and LVH between the subjects with and without PVT was significantly difference（P0.01）.Heart function stage Ⅲ-Ⅳ and low blood kalium had induced PVT easily.Conclusion Organic heart disease complicated with PVT was correlated to LVEDD,LVH,heart functional class and low blood kalium.
