摘 要:目的评价同源异体血小板凝胶促进糖尿病性难治创面愈合的疗效。方法采用同源异体单采血小板为原料制备血小板凝胶(platelet gel,PG)。32例患者分为皮肤溃疡型和窦道型2类,各类分别设立对照组与实验组,皮肤溃疡型各10例,窦道型各6例,对照组采用临床常规方法,实验组采用PG治疗,观察并记录创面愈合情况,测量愈合面积或体积,进行统计学分析。结果 32例糖尿病性溃疡患者综合疗效评价统计结果显示,糖尿病性皮肤表面溃疡型和窦道型溃疡种类型的PG治疗效果明显优于对照组,实验组12周痊愈率达到87.5%,而对照组仅为12.5%。皮肤溃疡型和窦道型创面治疗第3、6、12周时的愈合率实验组高于对照组。实验组具有疗程短、换药间隔时间长、愈合质量好等优势。结论同源异体血小板凝胶用于治疗糖尿病性难治性溃疡效果明显优于常规治疗方法,无不良反应,是一种简单易行、安全有效的治疗糖尿病性难治性创面愈合的方法。Objective The aim of the study was to evaluate the healing effects of allogeneicplatelet gel(PG) on diabetic refractory wounds.Methods PG was made from allogeneic single-donor platelet concentrations.A total of 32 patients including two types of wound,ulcer(n=20) and sinus(n=12),were equally divided into control group and experimental group.The control group was treated with routine method while the experimental group was treated with PG.The areas of ulcers and the volume of sinus were observed and measured.The results were analyzed using statistical software.Results The results showed that regardless of either the ulcer or sinus wound,experimental group has an advantage over the control group.The recovery rate of the experimental group was 87.5%,while the recovery rate of the control group was 12.5%.The accreting rate of experimental group,regardless of either ulcer or sinus wound,was significantly higher than that of control groups at week 3,6,12.Experimental group had an advantage over control group regarding the course of treatment,the interval of dressing change and healing quality.Conclusion The healing effects of allogeneic PG on diabetic refractory wounds were better than routine method,no adverse effects were observed in the treatment process.The use of allogeneic PG may provide a simple,safe,and effective means of treating diabetic refractory wounds.
