摘 要:目的:探讨定量感觉检查(Quantitative tative Sensory Testing,QST)在早期筛查糖尿病周围神经病变的临床意义。方法:分别对100例2型糖尿病患者进行肌电图(神经传导速度)和QST检查,以神经传导速度阳性为诊断糖尿病周围神经病变的金标准,探讨QST检查用于早期筛查糖尿病周围神经病变的灵敏度和特异度。结果:100例2型糖尿病患者中,神经传导速度阳性者有52例,阴性者有48例;同时QST检测法阳性者有95例,阴性者有5例。QST检测法的用于早期筛查糖尿病周围神经病变的灵敏度为98.18%,特异度为8.89%,阳性预测值为56.84%,阴性预测值为80%。结论:QST在糖尿病周围神经病变的早期筛查中灵敏度较高,但特异度较低,可作为常规肌电图的补充检查。Objective: To evaluate the clinical significance of Quantitative Sensory Testing, (QST) in screening of the early stage diabetic peripheral neuropathy. Methods: Inspecting 100 patients with type 2 diabetes by means of electromyography (nerve conduction velocity, NCV) and QST examination, respectively, and with the NCV positive as the gold criterion for screening of the early stage of diabetic peripheral neuropathy, and further analysis the sensitivity and specificity of QST for diagnosis of the early stage of diabetic peripheral neuropathy. Results: Among the 100 patients with type 2 diabetes, For NCV examination, there were 52 cases positive and 48 cases negative; On the other hand, for QST examination, there were 95 cases positive, and 5 cases negative. The sensitivity and specificity of QST examination for the diagnosis of early stage of diabetic peripheral neuropathy were 98.18% and 8.89%, respectively, and the positive predictive value was 56.84% and negative predictive value was 80% for QST, respectively. Conclusion: In the screening of early stage of diabetic peripheral neuropathy, QST showed higher detection sensitivity, but lower specificity than NCV examination. Therefore, QST probably be an examination for the supplement of the routine electromyography.
