摘 要:目的研究本院前列腺炎患者前列腺液的病原学及药敏情况,以协助临床前列腺炎合理的用药,来减少耐药菌株的产生。方法对临床诊断为前列腺炎患者的前列腺液进行细菌培养和218例支原体、衣原体培养,同时做药敏试验。普通培养采用血琼脂平皿、沙氏培养皿、L型培养皿;支原体采用解脲人型双向选择培养基,沙眼衣原体采用胶体金法。结果 218例前列腺炎患者普通培养中单一菌感染122例,混合感染1例,细菌感染阳性率56.4%;L型培养中与普通培养共同阳性且为同一种菌者76例,阳性率34.9%,单纯L型培养阳性者3例,阳性率1.4%;支原体、衣原体培养阳性46例,阳性率21.1%。凝固酶阴性葡萄球菌对万古霉素的敏感性中出现了1例中敏,仅万古霉素、利福平、庆大霉素、克林霉素、复方新诺明等5种药物的敏感性超过了50%,敏感率分别为98.5%、87.7%、60.0%、50.8%、50.8%;金黄色葡萄球菌对利福平、庆大霉素、氯霉素、强力霉素敏感性较高,敏感率分别为77.8%、66.7%、55.6%、50.0%。大肠埃希菌中对阿米卡星、磷霉素、美洛培南、哌拉西林、头孢类(包括头孢哌酮、头孢噻肟、头孢他啶、头孢呋肟)、喹诺酮类(包括环丙沙星、左氟沙星)、呋喃妥因等敏感性较高,敏感率分别为90.0%、90.0%、80.0%、60.0%、55.0%(60.0%、60.0%、50.0%、50.0%)、50.0%(50.0%、50.0%)、50.0%。结论葡萄球菌可能在前列腺炎中起一定作用,解脲、人型支原体和衣原体也可能有一定的致病作用,临床应根据培养和药敏的结果合理用药,以避免更多耐药菌株的产生。Objective To study the etiology of prostatic fluid and its drug sensitivity among prostatitis patients in the Third Xiangya Hospital of Central South University,to assist the rational use of antibiotics in clinical practice,and to reduce the produce of drug-resistant strains. Methods Bacterial culture of prostate fluid,mycoplssma and chlamydia culture were conducted in 218 patients clinically diagnosed with prostatitis,and drug susceptibility test was conducted simultaneously.Blood agar-agar plate,Shashi petri dishes,and L petri dishes were used to make the ordinary culture.Petri dishes of bi-directional choice of ureaplasma were used to make mycoplasma culture.Colloidal gold method was adopted to make Chlamydia trachomatis culture. Results Out of the total 218 cases,122 cases were found to have single pathogen involved in infection and 1 case had mixed infection.And the positive rate of bacterial infection was 56.4%.76 cases were found to be positive in both L-type culture and ordinary culture and the positive rate was 34.9%.3 positive cases were found in L-type culture,with the positive rate of 1.4%.In the process of mycoplasma and chlamydia culture,the positive was seen in 46 cases and the positive rate was 21.1%.The coagulase negative staphylococcus showed intermediate sensitivity to vancomycin and relatively more sensitive to antibiotics like vancomycin,rifampin,gentamicin,clindamyin,and compound sinomin,the sensitive rates were 98.5%,87.7%,60.0%,50.8%,and 50.8%,respectively.Staphylococcus aureus was more susceptible to rifampin,gentamicin,chloramphenicol and doxycycline,the sensitive rates were 77.8%,66.7%,55.6%,and 50.0%,respectively.Escherichia coli was more sensitive to amikacin,fosfomycin,meropenem,piperacillin, cephalosporins(cefoperazone,cefotaxime,cetazidime,cefuroxime),quinolones(ciprofloxacin,levofloxacin,) and nitrofurantoin,with the sensitive rates of 90.0%,90.0%,80.0%,60.0%,55.0%(60.0%,60.0%,50.0%,50.0%),50.0% (50.0%,50.0%),50.0%,respectively.
