SLIPA喉罩在妇科腹腔镜手术中的应用观察

摘　　要：目的：观察SLIPA喉罩在妇科腹腔镜手术中的应用效果及安全性。方法：60例ASA 1~2级择期妇科腹腔镜手术患者,随机分为SLIPA喉罩组（S组）和气管插管组（T组）各30人,记录入室时、插管（罩）前、后1 min和拔管（罩）前、后1 min的MAP、HR、SPO2,分别记录插管（罩）成功时、气腹后20min及手术结束时的Ppeak、PetCO2,记录反流误吸发生率及术后咽痛发生率。结果：S组各时点MAP、HR变化差异无统计学意义（P〉0.05）。T组插管后1 min、拔管前1 min、拔管后1 min与插管前1 min相比,MAP明显增高、HR明显增快,且与S组同时点相比,差异有统计学意义（P〈0.05）。两组间Ppeak、PetCO2变化无明显差异（P〉0.05）,术后咽痛发生率T组比S组高。结论：SLIPA喉罩用于妇科腹腔镜手术,通气满意,并发症少,是一种安全、有效的麻醉方法,值得临床推广。Objective： To observe effect and safety of SLIPA laryngeal mask in laparoscopic surgery.Methods： A total of 60 cases at 1-2 stage of ASA were randomly divided into SLIPA laryngeal mask group（Group S） and tracheal intubation group（Group T） by half.MAP,HR and SPO2 were recorded as entering larynx,1 min before and after incubation or masking,and Ppeak and PetCO2 were also recorded when the incubation or masking was successfully performed.The incidence of countercurrent mistake inhalation and postoperative pharyngalgia.Results： There were no significant differences in MAP and HR in Group S（P0.05）;while MAP and HR significantly increased comparing that at 1 min before intubation and that at the other times.The differences in MAP and HR between two groups were significant（P0.05）.There were no significant differences in Ppeak and PetCO2 between two groups（P0.05）.The incidence of pharyngalgia was higher in Group T.Conclusions： SLIPA laryngeal mask provides satisfactory ventilation.It is effective,safe,of little complication and is worthy clinical extensive application.
